The Discharge Callback Registered Nurse (RN), functions as an essential member of the emergency department team that reach out to patients seen in the emergency department (ED) and discharged home. Many patients treated and discharged home from the ED require clinical follow-up and referral to outpatient services. The Discharge Callback RN works to reduce gaps in care and identify opportunities to improve care provided in the ED.

Responsibilities
- 1. Utilizes the nursing process when reviewing the medical record of patients being followed up on by assessing for gaps in care and assisting in directing patients to appropriate follow-up.
- 2. Evaluates, Responds to & prioritizes requests for clinical follow-up phone call as consulted by ED staff.
- 3. Ensures patients are informed regarding diagnostic lab & imaging results that occur after the patient is discharged and directed to appropriate care follow-up.
- 4. Responds to patient and family questions and directs post ED patients to appropriate resources
- 5. Identifies opportunities for improvement of ED care and escalates concerns to nursing / physician leadership teams.
- 6. Attempts contact to any patient who left prior to being seen or left against medical advice from the ED
- 7. Facilitates / coordinates primary care & specialty resources and appointments available to ED patients.
- 8. Acts as a content expert by assisting with ongoing education to ED staff to improve discharge planning and education.
- 9. Implements and role models the Standards of Professional Behavior when interaction with patients and families
Qualifications
EDUCATION
Graduate of an accredited school of nursing (Diploma, AD, BS, BSN preferred )
EXPERIENCE
Miinimum 2 years Emergency Nursing Experience
LICENSURE
RN In State of CT, Certification in Emergency Nursing preferred
SPECIAL SKILLS
Computer skills, strong service excellence telephone skills, proficient in electronic medical record & completing chart review
PHYSICAL DEMAND
May be exposed to infectious diseases. Moderate stress. Light Duty.
Additional Information
2+ years of Emergency Nursing experience is required.
