RN DIABETIC NURSE
A DAY IN THE LIFE OF A REGISTERED NURSE DIABETIC NURSE
As a full-time RN Diabetic Nurse, you play a critical role in the provision and coordination of care for our home health care agency and the facilities. You are responsible for preventing adverse events from happening as well as optimizing our health service productivity and patient outcomes. Keeping our organization's mission, vision, and values at the forefront of your mind, you facilitate the delivery of top-quality, patient-centered care while going through your daily tasks. Your tasks fluctuate based on your specific client caseload and schedule. Primarily, you provide direct patient care to acute and chronically ill patients in the comfort of their assisted living facility.
In addition, you complete initial and outgoing comprehensive assessments of the patients' needs, including OASIS assessments at the appropriate point in time. Initiates the POC and revises as necessary under the direction of a physician. Provides those services and/or treatments requiring substantial specialized nursing skill. Assesses and educates the patient and family/caregiver regarding the disease process, self-care techniques, prevention strategies, and nursing-related needs. Prepares clinical and progress notes. Supervises LPN's, and home health involved in the patients care and gives them education regarding diabetes. Performs record reviews and assesses current diabetic treatment regimen and work with provider on the best treatment regimen and adjust as needed. Work with Assistant Clinical Manager and assist with referrals, task screens, and recert decisions as needed. Reconcile patients' medications during each visit. Informs the physician and other staff of changes in the patient's condition and needs. Evaluates the effectiveness and outcomes of care. Participates in Administrative on-call. Maintains regular attendance and punctuality which are critical in order to complete the day-to-day tasks of this position.
QUALIFICATIONS FOR A REGISTERED NURSE DIABETIC NURSE
- Current RN license
- Emotional intelligence and the ability to show compassion and empathy toward others.
- Ability to overcome challenges and defuse conflict.
- Experience of being a Diabetic Nurse
