Registered Nurse - Clinic Rn - Float

Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As a Clinic RN, you will provide nursing care for patients in our clinic setting. You will assess, plan, direct and evaluate nursing care for assigned patients, as determined by the patient’s physical, psychosocial, and cultural needs in accordance with established standards through the Iowa Board of Nursing and PHC policies and procedures.

• Like variety? This position may be for you! This position will float mainly between PHC @ Mercy Family Practice Residency Clinic, East Side Clinic, and West Side Clinic. As a float, you may work in any area of our clinics: family practice, OB/GYN, internal medicine, pediatrics, etc.
• Work collaboratively with a multidisciplinary team to welcome patients to a safe and inclusive environment and to engage patients in holistic care.
• Typical schedule is Monday - Friday 8 AM - 5 PM with some evenings until 8 PM and approximately every 6th Saturday 9 AM - 1 PM. Flexible scheduling may be available.
• Earn a shift differential when you work at least a 3 hour shift either in the evenings after 5 PM or anytime on a Saturday.

What You Will Do

1. Provides leadership to the care team to ensure efficient patient flow and completion of daily tasks. Assigns care team members in accordance with patient needs and staff capabilities and qualifications to meet the needs of the clinic. Participates in performance management of the care team support staff. (Care team coordination)
2. Provides population health management for empaneled patients to assist with the chronic care management and addressing gaps in care. (Population health management)
3. Performs quality assurance reviews for life safety controls/tasks and duties assigned to clinical support staff. (Quality control)
4. Triages patients in-person or on the phone according to established protocol. (Patient triage)
5. Coordinates the patient medication management ensuring comprehensive medication reconciliation, appropriate patient education, and that medication refills are completed per standing orders. (Medication refills)
6. Documents patient care accurately and timely in the electronic health record. (Clinical documentation)
7. Assists clinical support staff as needed, including but not limited to, facilitating patient rooming and intake, obtaining vital signs; performing point-of-care, lab, and diagnostic testing per orders; administering medications, immunizations, and treatments per orders, completing prior authorizations and referrals to ensure efficient care team workflow. 8. Serves as a patient advocate, resource, and point-of-contact for patients through the continuum of care. Provides care management of patients defined by the health center as high risk and/or needing education.
9. Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures.

Qualifications You Need to Bring

• Completion of an accredited RN program accepted by the Iowa Board of Nursing for licensure
• Excellent customer service skills.
• Effective English communication skills, written and verbal.
• Knowledgeable of nursing care regulations and practices established by the Iowa Board of Nursing.
• Demonstrates competency of required nursing skills.

• Licenses & Certifications:
  • Current RN license accepted by the State of Iowa Board of Nursing
    
  • *Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the State of Iowa
  • *Basic Life Support (BLS) for Health Care Providers Certification recognized by the American Heart Association.

Your experience and skills determine your base pay. The hiring range for this position is typically $25.00 - $31.00 per hour, though highly experienced candidates may earn up to $36.00 per hour. This position is not eligible for benefits.
