General Responsibilities:
- Contribute to increasing participants’ satisfaction by developing and maintaining positive relationships with co-workers, participants and their families/significant others, and members of the community.
- Provide nursing care for participants in the Center, Clinic and at their homes to promote safe, high-quality service 24/7.
- Evaluate the competence of all CNAs and CHHAs initially during orientation and on an ongoing basis in accordance with policy.
- Complete timely and accurate nursing assessments in accordance with policies and requirements of the state.
- Participate in 24/7 “on-call” process for triage of participants and their needs.
- Work collaboratively with other team members, especially the interdisciplinary team to develop and implement the nursing and home care components of the comprehensive interdisciplinary team plan of care
- Function as RN in clinic; assisting participants, physicians &/or NPs with clinic visits, appointments, and care
- Maintains the clinic and assessment room with appropriate supplies and equipment.
- Ensures all infection control practices are adhered to by all participants and staff in the center and clinic
- Participate in all interdisciplinary team meetings:
- Complete initial RN assessment and reassessment face to face as part of the interdisciplinary team
- Contribute to the care plan for nursing needs by the participants.
- Review and revises goals and approaches to participants care in coordination with interdisciplinary team.
- Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant’s health problems.
- Communicate with other interdisciplinary team members to coordinate follow up of nursing interventions.
- Monitor effectiveness and outcomes regularly and keep the team informed as to participant progress and status regarding level of service.
- Assesses the participant /family understanding and capacity to manage self-care or family provided care.
- Assist in developing and maintaining written job descriptions for nursing personnel, and assigning duties based on education, training, competencies, and job descriptions.
- Participate in family meetings, staff meetings, in-service and training and orientation programs as required.
- Follow all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, participants, and families.
- Practice standard precautions and follow PACE Program Infection Control protocols.
- Perform other duties as required or requested.
