We are looking for an outstanding Registered Nurse (RN) to join our team in League City/ Galveston, TX.

Core Responsibilities:
- Conduct assessments of incoming patients
- Manage your own case load and conduct visits where patient resides
- Provide symptom management and oversee patient’s plan or care while collaborating with Hospice team members
- Participate in anon-call rotation for evenings and weekends
- Have knowledge and understanding of all services hospice provides and keep patient and family informed of all benefits available
Day to Day Activities:
- Educate patient and family on disease progression, make visits, care coordination and management of patient and family needs related to Hospice diagnosis
- Deliver outstanding bedside care
- Foster relationships with your patients and families to make a difference in their greatest time of need
Who the Job reports to:
- Director of Clinical Services
Requirements/ Qualifications:
- Valid Registered Nurse (RN) license
- 1-2 years of nursing experience (preferred)
- Driver’s license, insurance and vehicle
- Utmost compassion and integrity
