Registered Nurse Case Manager

Cape Fox subsidiary Concentric Methods, LLC is accepting resumes for a Registered Nurse Case Manager to work full time providing Outpatient Medical Management under the Medical Group (Occupational Health) to our deserving Air Force at Warner Robins AFB, Georgia. The Registered Nurse Case Manager will work within an outpatient health facility.

A minimum of 3 years for ADN or 2 years for BSN full-time experience in clinical case management working with adults.

Identify and stratify individual patients and populations of patients across the spectrum of care at risk for chronic diseases and co-morbid conditions and evaluate the incidence of such conditions from a variety of established data bases and facility treatment team referrals.

Facilitate identification, adoption, implementation and utilization of standardized clinical practice guidelines and protocols for management of specific diseases/conditions.

Conduct a thorough evaluation of the patient’s current physical, psychosocial and health status. Document treatment plans and matches level of patient’s care to clinical risk, readiness to change and health literacy.

Educate/train team/facility staff on policies and procedures for utilization of guidelines, protocols and other case management activities. Fosters collaboration and facilitates case management activities with the patient care team.

Identify, collect, interpret and evaluate measurable outcomes of care within established time frames (i.e., quality of services, cost and cost savings, case management and improvement).

Conduct special studies as indicated based on outcomes and other Quality Management/Risk Management and Population Health programs and indicators.

Report data as required in coordination with the Health Care Integrator and Chief of Medical Staff. Report disease management efforts to MTF leadership through committees such as Population Health Working Group

Recognize adverse signs and symptoms and quickly reacts in emergency situations.

Complete in-person and telephonic evaluations that include determination of optimal time and location for patient management and follow-up care.

Prescribe and communicate treatment plan and patient teaching in accordance with established protocols. Collect and assess significant patient history information and perform all necessary patient teaching.

Demonstrate skill in setting up, operating and monitoring specialized medical equipment such as cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, and other clinic specific equipment.

Complete medical record documentation and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies and guidance.

Perform physical exam and health histories.

Administer medications, wound care, and numerous other personalized interventions.

Identify, develop and execute appropriate interventions such as, but not limited to, individual and/or group patient contacts and training classes.

Assists in development of facility’s Medical Management/Population Health plan in support of the annual Business Plan, ensuring a process for collecting disease management data, monitoring performance, and identifying improvement opportunities.

• Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE) with a degree in Associate’s Degree in Nursing OR Bachelor of Science in Nursing.
• A minimum of 3 years for ADN or 2 years for BSN full-time experience in clinical case management working with adults.
• Current, full, active, and unrestricted license to practice as a Registered Nurse in the state where the nurse practices.
• Basic Life Support from a nationally accredited agency (such as American Heart Association or American Red Cross).
• Proficient in Microsoft Office (Word, Excel, Access, PowerPoint, and Project).
• Must have authorization to work in the United States as defined by the Immigration Reform Act of 1986
