Job Description:
We are seeking a dedicated and compassionate Registered Nurse to fill the position of Case Manager at our medical facility in Rochester, NY. As a Case Manager, you will play a vital role in ensuring high-quality and holistic care for our patients while also managing their overall healthcare experience.
Responsibilities:
- Serve as an advocate for patients, ensuring their needs are met and their healthcare goals are achieved. Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans based on patients' medical history, current conditions, and overall healthcare needs.
- Oversee and coordinate patient care, ensuring seamless transitions between healthcare providers and services. Collaborate with healthcare professionals, including physicians, nurses, social workers, and therapists, to ensure effective communication and coordination of care.
- Conduct thorough assessments of patients' physical, psychological, emotional, and social needs to develop comprehensive care plans. Monitor patients' progress, evaluate the effectiveness of interventions, and make appropriate adjustments to the care plans as needed.
- Facilitate access to healthcare resources, including medical equipment, medications, specialized treatments, and community support services. Assist patients in navigating the healthcare system, including insurance and financial matters.
- Provide patients and their families with education and support to enhance their understanding of their medical conditions, treatment options, and self-care techniques. Empower patients to actively participate in their care and make informed decisions about their health.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes. Prepare and present comprehensive reports on patients' progress and treatment outcomes to healthcare teams and insurance providers as required.
Requirements:
- Registered Nurse (RN) license in the state of New York.
- Minimum of 2 years of clinical experience as an RN, preferably in a hospital or acute care setting. Case management experience is a plus.
- Strong clinical knowledge and understanding of medical conditions, treatments, and healthcare protocols.
- Excellent communication and interpersonal skills to effectively collaborate with patients, healthcare teams, and external stakeholders.
- Ability to work independently and manage multiple tasks efficiently in a fast-paced environment.
- Proficient computer skills for documentation, report generation, and communication.
