We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and providing comprehensive care to patients, ensuring their needs are met throughout their healthcare journey. This is a rewarding role that requires strong clinical knowledge, excellent communication skills, and the ability to collaborate with interdisciplinary teams.
Responsibilities:
- Conduct utilization management to ensure appropriate use of healthcare resources
- Perform case management duties, including assessing patient needs, developing care plans, and coordinating services
- Collaborate with healthcare providers to facilitate timely and effective discharge planning
- Monitor and document vital signs, symptoms, and changes in patient condition
- Provide specialized care in areas such as ICU, managed care, behavioral health, primary care, etc.
- Maintain accurate and up-to-date medical records
- Administer medications and treatments as prescribed by physicians
- Educate patients and their families on disease management and self-care techniques
- Advocate for patients' rights and ensure their preferences are respected
Skills:
- Strong knowledge of utilization management principles and practices
- Proficient in case management techniques and tools
- Excellent assessment and critical thinking skills
- Ability to effectively communicate with patients, families, and healthcare professionals
- Familiarity with medical records systems and documentation requirements
- Solid understanding of patient care standards and protocols
Qualifications:
- Valid Registered Nurse (RN) license in [state]
