Education- Completion of a BSN, or an ADN from an accredited institution.

Experience- Two years ambulatory nursing experience with BSN or three years with an ADN preferred, in areas such as public health, community health, or equivalent and experience in multicultural nursing or other social service area and commitment to community health care preferred. Care management or chronic disease management experience preferred.
