Summary
This part-time position assesses and treats patients utilizing standing and direct orders from physicians and other licensed practitioners within the scope of their practice, oversees tasks of the Medical Assistants, oversight of licensed vocational nurses (LVN), collaborates with street health and recuperative care teams, and provides patient care coordination with both external and internal providers.
Essential Functions
- Assisting with the direct care (triaging, administering medications, education, etc.) of patients and working with licensed clinicians in an outpatient clinic setting.
- Places and documents phone calls related to the coordination of patient care in an electronic health records system.
- Collects patient care metrics for process and quality improvement measures.
- Provides medication management, education, care coordination, health evaluation, wound care, and outreach, as needed, to improve patient outcomes.
- Documents all visits and medication administration in electronic health record and consults with patient care team regarding clients.
- Assists with processing referrals for recuperative care intakes, determining eligibility, intake assessment, care plan, ongoing assessment, and discharge planning, as needed.
- Coordinates with Village staff to ensure appropriate care coordination for recuperative care clients.
- Multi-task, communication with patients/de-escalation & assessment skills, provides emergency response, customer service, and trauma-informed care to provide optimal patient care.
- Provide oversight and training to newly licensed RN's and LVN’s, as needed.
- On-time completion of assigned training and policies.
- Performs other duties as assigned.
Qualifications
- Degree from an accredited nursing school
- Active RN license from Board of Registered Nursing in California or registered to take the licensing exam (NCLEX).
- CPR/First Aid & AED certified.
- Intermediate user of MS Office.
- Participate in an annual Tuberculosis screening and/or other screenings when necessary. Reasonable accommodations may be granted where appropriate
- Six months experience with electronic health record preferred.
