Requirements include:

• A current Registered Nursing license for the State of Rhode Island
• Prior clinical experience, preferably in home care with a working knowledge of OASIS
• Knowledge of Medicare and Medicaid guidelines a plus
• The ability to work independently to both assess and treat clients in home settings
• To educate the client, their families and their designated loved ones
• To communicate effectively with physicians, case managers and other professionals to coordinate care and offer reports as required
• Basic computer skills
• Current vaccinations as required by governmental agencies
• Other documents as required

Responsibilities include:

• Perform tasks as assigned
• Developing patient plan of care
• Supervising certified nursing assistant
• Managing patient care and communicating with physicians
