Only Angels Home Healthcare, LLC in Wichita, KS is looking for two Registered Nurse to join our 6 person strong team. We are located on 1624 S Rutan Ave. Our ideal candidate is a self-starter, ambitious, and hard-working.
Registered Nurse
Monday to Friday
$150 admission
$120 recertification
$45-50 per visit
$23 in office
Plus Bonus
Duties
To provide care in accordance with the patient's care plan to include comprehensive health and psychosocial evaluation, monitoring of the patients condition, health promotion and prevention coordination of services, teaching and training activities, and direct nursing care.
Responsibilities
- Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
- Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
- Prepare and present the patient's record to the clinical record committee.
- Consult with the attending physician concerning alterations to the patient care plan, check with the appropriate supervisor, and make changes.
- Coordinate patient services.
- Submit clinical notes no less often than weekly and progress notes and other clinical record forms outlining the services rendered.
- Submit a tally of patient care visits made each day.
- Participate in case conferences.
- Discuss with the appropriate supervisor the need for the involvement of other members of the home health team.
- Obtain orders for home health aide service and submit a referral to the appropriate personnel.
- Participate in the patient's discharge planning.
- Participate in the development and periodic revision of the physicians plan of treatment and process change orders.
- Maintain an ongoing knowledge of drug therapy.
- Adhere to federal, state, and accreditation requirements including Medicare and Medicaid regulations.
- May be requested by the Clinical manager to fill in for other nurses.
Coordinates the Admission of a Patient to the company.
- Conduct an initial and ongoing comprehensive assessment of the patients needs including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
- Obtain a medical history from the patient and/or family member, particularly as it relates to the present condition.
- Conduct a physical examination of the patient including vital signs, physical assessment, mental status, appetite, and type of diet etc.
- Evaluate the Patient, family member and home situation to determine what health teaching will be required.
- Evaluate the patients environment to determine what assistance will be available from family members in caring for the patient.
- Explain nursing and other company services to patients and families as art of planning for care.
- Develop and implement nursing care plan.
Provide Skilled Nursing Care as outlined in the nursing care plan.
- Nursing services, Treatments, and Preventative and rehabilitation nursing procedures for the patients care and safety
- The initiation of preventative and rehab nursing procedures for patients care and safety.
- Observing signs and symptoms and reporting to the physician: reactions to treatments, including drugs as well as changes in the patients physical or emotional condition.
- Teaching, Supervising and Counseling the patient and caregivers regarding the nursing care needs and other related problems of the patient at home.
- Consult with the attending physician concerning alteration of the plan of treatment in consultation with the supervisor.
Assumes responsibility for the care given by the Home Health Aide.
- Prepare the care plan for the Home Health Aide.
- Supervise and evaluate the care given by the Home Health Aide per company policy.
- Submit a written evaluation of the Home Health Aide who is providing service to the patient in their geographical area to the appropriate department.
- Participate in periodic conferences with the Home Health Aide supervisor concerning the aide's performance.
- Chart services rendered Change notes in the patients condition and/or family and home situation, and make revisions in the nursing care plan as needed.
- Provide guidance and supervision to the LPN and supervise the LPN per company policy. Participate in the planning, operation, and evaluation of the nursing service.
Job Conditions
- Must have a Drivers License and be willing and able to drive to patients residences.
- The ability to drive and the ability to access patients homes which may not be routinely wheelchair accessible is required.
Environment
- Contact with clients under a wide variety of circumstances.
- Care is provided in clients' living environments with varying situations including Private Homes, Independent or Assisted Living, or in an institution such as a hospital or Long-Term Care Facility.
- May be required to respond to emergency situations.
- Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious materials.
Physical Requirements
- On occasion may be required to bend, stoop, reach, and move patients that weigh up to 250 pounds. Lift and/or carry up to 30 pounds.
- Must be able to effectively communicate in English.
- Hearing, Eyesight, and Physical dexterity must be sufficient to perform a physical assessment of the patients condition and to perform patient care.
Qualifications
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in the state as a Registered Nurse.
- One or more years of experience in a community/home health company setting is preferred.
- Must have knowledge of Medicare and Medicaid guidelines.
- Must have a working knowledge of home healthcare and the principles and techniques of professional nursing and the required documentation that pertains to it.
- Should be skillful in organization and in the principles of time management and have knowledge of management processes.
- Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.
- Must be willing to undergo a background check.
- Must have CPR certification. Online CPR certification with not be accepted.
Available shifts and compensation: We have available shifts all days of the week. Compensation depends on your experience.
