  What youll be doing (not a complete list):
• Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
• Developing an individualized plan of care for each client with them, their family and the caregiver(s)
• Training, Supervising and Delegating tasks to staff as appropriate.
• Maintaining client records showing systematic assessments, planning interventions, and evaluations.
• Communicate with Physicians, Nurses and Initiating referrals as needed to other health team members.
  What were looking for:
• Current unencumbered license as a Registered Nurse (RN or BSN ok) in Oregon.
• Current CPR certification
• Access to reliable transportation and a cell phone.
• Maintains confidentiality in all aspects of the job.
• Knowledge of safety and infection control practices.
• Ability to learn our software and care planning process,
• Ability to learn and apply the Oregon Administrative Rules that we work under

  This job is part time, flexible hours, with additional hours adding to full time - or the option to hire on an additional part time RN as growth happens.
