Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time RN for our Community Support Program (CSP). CSP nurses work as a part of an interdisciplinary treatment team providing services to adults with severe and persistent mental illness.
Responsibilities

• Conduct comprehensive nursing assessments as part of a multi-disciplinary treatment team - initial health screenings and follow-up nursing appointments.
• Consult with Riverbend providers, team members, family members, significant others, and community providers, as needed, to support clients on their recovery journey.
• Maintain your own caseload and assist with other RN caseloads, as needed.
• Medication management - provide client education, administer medication (long acting injectables), refill management, prior authorizations, and consultations with pharmacies.
• Attends treatment team meetings and other meetings as required.
• Utilize electronic medical record (EHR) to document patient information, referrals or other relevant information as required.
• Adhere to all compliance procedures relevant to protected health information (PHI) and HIPAA regulations.

Requirements

• Valid New Hampshire Registered Nurse License
• At least one-year nursing experience required
• Certification in Basic Life Support
• Experience working with individuals with psychiatric illnesses preferred
• ANCC board certification in Psychiatric Mental Health Nursing preferred
• Valid driver's license and personal vehicle or access to a reasonable alternative is preferred
