The responsibility of this position is to provide professional nursing service in the care and treatment of patients in a comprehensive health care setting under the direct supervision of the Senior Community Health Nursing Supervisor and coordination with another Registered Nurse. Participates in clinical and clerical functions.

All of the following duties require accuracy, safety, positive customer and team interactions, and adherence to protocol, polices, and procedures at all times. A current Florida Nursing License in accordance with Florida Statue 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27. Duties will include, but are not limited to: Public Health Competency Domain 2: Policy development/Program planning skills + Assist with collecting, compiling, and analyzing client data for weekly, monthly, and annual program and/or agency reports.

Public Health Competency Domain 3: Communication skills Public Health Competency Domain 4: Cultural Competency skills Public Health Competency Domain 5: Community Dimensions of Practice Skills + Coordinates and links clients with other health and/or social service agencies to facilitate movement of clients through the public health system. + Serves as a liaison between clients in need of care, and other service agencies in the community. In particular, serves as a key liaison between other healthcare providers, including hospitals, private medical providers and other public healthcare facilities, and DOH-Duval.

+ Conducts reminder calls to scheduled patients. Conducts calls to patients who didn't show for their appointments to reschedule them. Makes notes on the schedule for patient responses and keep records of the data.

Public Health Competency Domain 6: Public Health Sciences skills + Maintains awareness of patient's allergies. + Accurately documents all clinically pertinent information. + Administers tuberculosis screening in conjunction with Shelter Card Program.

+ Administers tuberculin skin tests (TSTs). + Provides Direct Observed Therapy (DOT) to patients on therapy when needed. + Creates and maintains patient records, reports, and forms in a timely manner.

+ Correctly documents patient's health status. + Assists providers in clinic sessions, maintains medical records, interprets, and implements provider's orders/recommendations to patient, assists with patient care and follow up as appropriate. + Ensures information and communication with patients and patient's support system are positive, accurate, and clear.

+ Takes initiative to listen and understands the views of others. + Manages active TB cases and coordinates care for the case and close contacts. + Manages medications for active TB cases and their close contacts.

+ Accurately applies nursing principles and techniques in the direct care and treatment of active TB patients and their close contacts. + Provides direct patient care along with in-depth teaching, guidance, counseling, and advisement that is directed toward the prevention, promotion, and maintenance of optimal health. + Develops, implements, monitors, and modifies plans of case for individuals in various settings though an interdisciplinary and collaborative process with other members of the TB Program team.

+ Provides health education. Public Health Competency Domain 8: Leadership and Systems Thinking skills. + Participates in educational, professional, and community activities that relate to professional growth and aids in the improvement of the delivery of client care.

+ Attends clinical staff meetings, in-services, seminars and trainings for self-improvement, increased competency, and retention of skills. + Conducts TB related community presentations that promotes and increases public awareness of the need and benefits of participation in health promotion/prevention activities. + Works with the management team, physicians, nurses, health support technicians, human service counselors, senior clerks, and other staff to develop and provide an interdisciplinary team approach to case management.

+ Participates in quality improvement activities. + Ensures timely completion of all mandatory trainings. + Ensures accurate and timely submission of employee leave and attendance records, Employee Activity Records (EAR's) and travel vouchers.

+ Performs other related duties as assigned. Required Knowledge, Skills, and Abilities: + Knowledge of nursing principles and techniques + Knowledge of human anatomy and physiology. + Skill in using medical instruments or testing equipment used in providing nursing care.

+ Ability to demonstrate manual dexterity. + Ability to administer medications. + Ability to work with patients.

+ Ability to interview patients and obtain a complete medical history. + Ability to interpret and follow medical instructions. + Ability to observe patient's symptoms and reactions to treatments.

+ Ability to communicate effectively. + Ability to establish and maintain effective working relationships with others. + Current knowledge in tuberculosis transmission, prevention, and treatment.

+ Ability to teach healthcare.
