Job Description
Provides daily supervision of CHHA, and PCT, and ensures the quality of service is within the required plan of care
· Initiates and reviews the clients, medical history, medications, and other statuses from client records
· Assists with the planning, coordination, leading, controlling, and evaluating of home health services
· Identifies and evaluates client needs and appropriateness of home care·
· Assists with the orientation of new professional staff to familiarize them with company policies and procedures
· Conducts client visits as needed
· Initiates in-home supervisory visits of clinical staff per state requirements
· Maintains confidentiality of medical records, personnel records, etc.,
· Works closely with the electronic medical record to ensure client electronic charts are maintained, updated, and remain in compliance with company standards
· Provides trainer sessions for peers and field staff to expand training team expertise
· Provides coaching support to new staff to ensure compliance and consistency
- Registered nurse in good standing, holding a Bachelor of Science degree in nursing.
- Knowledge and experience in a home care setting
- Ability to demonstrate self-confidence and positive attitude toward self and others, and maintain commitment and enthusiasm to achieve goals
- Ability to identify and evaluate personal strengths and weaknesses
- Customer service-oriented
- Knowledge of, Microsoft office applications
- The ability to communicate in English well, both verbally and in writing
- Bilingual (preferable)
- Willing to travel
May be required to bend, stoop, reach, and move clients as well as lift and carry more than 50 pounds
