• Compensation: $40 - $80 per hour

Key Responsibilities:

• Conduct initial and ongoing comprehensive assessments, including OASIS assessments.
• Develop, implement, and revise personalized care plans based on patient needs.
• Provide specialized nursing care with skill and compassion.
• Counsel patients and families, empowering them with healthcare knowledge.
• Administer preventive and rehabilitative nursing practices.
• Maintain detailed and accurate clinical progress notes.
• Oversee and coordinate patient care, including on-call rotation schedule.
• Supervise nursing and paraprofessional staff to ensure high-quality care.
• Ensure compliance with Medicare/CHAP/OSHA guidelines and Agency policies.
• Demonstrate professional comportment in appearance and interpersonal relations.

Job Requirements:

• Valid Registered Nurse license in Illinois.
• Minimum 2 years of experience in Home Health nursing.
• Proficiency in Axxess software is a must.
• Knowledgeable in Start of Care procedures.
• Current CPR certification.
• Up-to-date Tuberculosis test results.
