Job Summary and Qualifications

This position supports the mission of Presbyterian/St Luke's Medical Center of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Certified Diabetic Specialist is responsible for advanced nutrition care of patients who require specialized nutrition support and diabetes management. Will develop and implement nutrition care plans based on the nutritional and educational needs of the individual patients, with specific emphasis on assessment, development of a therapeutic plan, implementation, monitoring and documentation of the patient's response to specialized nutrition support and diabetes management. Communicates with medical, nursing staff, FANS employees, patients and families, and other health care professionals to assure that individual patient care needs are met. When need arises, will perform other duties as assigned by supervisor.

What you will do in this role:

  • The P/SLMC & RMHC Diabetes Self-Management Education & Support Center (DSME-SC) RDN who is also a Certified Diabetes Care & Education Specialist.
  • He/she is responsible for the planning, implementation and evaluation of all Diabetes Self-Management and Support services.
  • This position acts as a liaison to other departments and administration, Monitors and facilitates maintenance of all DSMES team members qualifications (CE credits, training, competency, licensures and registration), Maintains the ADA recognition or ADCES Accreditation and participates in the evaluation of DSMES effectiveness.
  • This position also functions as one of the DSMES team members and participates in the assessment, development and implementation of the plan of care for inpatients and outpatients patients with diabetes and any co-morbidities.
  • When need arises, will perform other duties as assigned by supervisor.

What qualifications you will need:

  • Current Registered Dietitian. National certification as a Diabetes Educator preferred.
  • BS, BA and/or master degree from accredited college or University in Foods and Nutrition or Dietetics. Prefer ongoing CEU's in specialty area of practice, and as appropriate be active in local, regional and national education and research programs.
  • Five or more years of diabetes related clinical experience.
