Description

STRIDE Community Health Center is a 501(c)(3) Federally Qualified Health Center (FQHC) with a 34-year history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE Community Health Center has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education, care coordination, patient navigation, pharmacy, and outreach and enrollment services in accordance with its mission.

What will you be doing in this role? A successful candidate will:

General Purpose:. The Refugee Clinic Coordinator also facilitates and expands STRIDEs external partnerships with resettlement agencies and mental health services. The Refugee Clinic Coordinator will assist with preparing, facilitating, and coordinating weekly screening appointments by creating patient charts, scheduling interpreters, and scheduling providers.

Essential Duties/Responsibilities:

Clinic coordination

  • Coordinates the appropriate access to physical and behavioral health services to patients, including collaborating with clinical and other care team staff to address patients needs:
    • Performs patient outreach via phone and in-person to schedule and re-schedule appointments.
    • Connects refugee patients to specialty care and Care Coordination.
  • Provides timely documentation and required reports for all activities.
  • Work closely with medical and care team to help ensure patients have comprehensive and coordinated care.

Screening Coordination:

  • Manages Red Cap
  • Schedules and facilitates all medical and BHP screening appointments- pre-screening, Screening Day1, Screening Day2 (SD1, SD2)
  • Schedules and facilitates interpretation services for screening days.
  • Coordinates and communicates closely with resettlement agencies about screening day logistics (ie. scheduling, transportation, interpretation services etc.) and other patient related needs.
  • Coordinates and communicates closely with internal staff related to screening day preparations.
  • Communicates closely with CDPHE and CRSP partners regarding all referral cases.
  • Ensures appropriate ROIs and resettlement

Non-Essential Duties/Responsibilities

  • Attends all staff meetings and all other meetings as assigned.

Material and equipment directly used: Skill in using computer software including but not limited to the internetand MS Office Products.

Working environment/physical activities: Office environment within a clinical setting. This is a grant funded position and may be subject to change dependent on funding. Enters data into computer programs via computer, mouse and keyboard. Moves about the office environment and occasionally to other locations. Moves/transports objects up to 25 lbs occasionally. Communicates information to others. Discerns/analyzes information from others to assist in decision making. Ability to do patient home and area hospital visits, as required under project or grant. Position is grant-funded.

Qualifications

Education and Experience

  • High school diploma or GED required.
  • 2+ years' experience in office administration/coordination
  • Advanced practical knowledge of Microsoft Office, including Outlook, Excel, Word and PowerPoint required.
  • Experience within the healthcare or non-profit industry preferred.
  • Fluency in another language preferred.
  • Previous experience working in social work, behavioral health or community health preferred
  • Background in clinic coordination, preferred
  • Experience working with culturally diverse and underserved populations.
