About the Job

FHR currently has vacancies for applicants with different levels of experience and education as either a Direct Care Worker or Case Manager. Staff serve as a member of a team in building relationships with individuals and developing shared understandings of the individuals’ goals as well as the steps necessary to achieve those goals. Staff deliver interventions identified on the Treatment Plan. Administer medications per our Medication Administration Policy. Provide assessment of needs, intensive clinical interventions and recovery tools, in a therapeutic setting or supporting individuals living independently in the community to promote stability.

Responsibilities

· Managing a caseload of assigned program individuals

· Assisting individuals and program staff with tracking and administering medications

· Educating individuals in the program about medicines and general medical advice

· Communicating with program staff and community partners about general public health trends observed

Qualifications

· High School Diploma or GED and one year full-time experience in providing direct care service to mentally ill individuals preferred for a the Recovery Support / Direct Care Worker position

· Preference will be given to multi–lingual candidates who meet the cultural and linguistic needs of the population served

· Staff must obtain MAP certification within first six months of employment
