Description
POSITION DESCRIPTION
Position: REACH Program Case Manager
General Purpose:
Under the supervision of the Director of Intervention & Diversion Services, the Case Manager will deliver re-entry services to program participants, in coordination with Juvenile Justice entities and Community Support Services. The Case Manager is responsible for the creation and implementation of re-entry plans for 30-45 participants, annually. Re-entry services include meeting with the REACH members and parent(s) or guardian(s) for a minimum of one calendar year.
The primary goal of this position is to ensure that there is "No Wrong Door" for participants in need of services.
Essential Duties & Responsibilities
Completes intake assessments for readiness, skills, strengths, aptitude and any potential barriers to being academically successful, demonstrating good character and living a healthy lifestyle.
Develops an Individual REACH Plan (IRP) within 30 days of release, to include goals for growth within the individual, family, school, work and community components for each participant. Identifies and maintains collaborative relationships within the community to support IRPs.
Conducts intensive case management and as needed, crisis intervention. Coordinates counseling & therapy services, wellness & recovery practices, comprehensive plans for reducing family and environmental stressors and criminogenic behaviors, post release support groups, home and community visits to reduce barriers to participation.
Ensures participants attend weekly meetings with the goal of being securely housed, employed and pursuing training or educational goals within 45 days of release. Documents services provided in a thorough, accurate and timely manner.
Prepares reports and recommendations regarding each participant's progress, ongoing needs and outcome measures.
Serves as a role model to participants by believing in the ability of every youth to succeed & expressing that belief. Fosters an environment that facilitates the achievement of Academic Success, Healthy Lifestyles & Good Character. Maintains an unwavering enthusiasm and a positive attitude with youth and within the Club community. Conveys warmth, care & proper guidance in ongoing interactions with participants.
Creates and implements Club programming in Juvenile Hall as needed.
Other
Abides by the policies and procedures of Boys & Girls Clubs of Sonoma-Marin and Juvenile Hall.
Attends staff meetings, community meetings and Club sponsored events as required.
Attends local training and workshops in the field.
Abides by Federal and State Laws.
Provides quality customer service by meeting and greeting the public, parents, members and outside groups who enter the facility. Conducts guided tours and answer questions.
Wears appropriate uniform while working at and/or attending Boys & Girls Clubs of Sonoma-Marin functions.
Responsible for participating in team and organization wide recruiting efforts.
Performs other duties as assigned.

Requirements
Education & Experience
Four-year degree in related field from an accredited college or university, preferred.
Experience working with troubled youth strongly preferred.
Computer skills: Microsoft Office.
Other Skills
Person will be of flawless integrity.
Bi-lingual; Spanish/English Strongly Preferred.
Group leadership skills, including an understanding of group dynamics.
Requirements
Complete and pass a criminal history and background check which includes but may not be limited to; Fingerprint Clearance, Review of DMV Records, as well as reference checks conducted by both Club staff and Juvenile Hall/Probation Staff.
Mandatory negative TB test.
Must provide own vehicle, have a valid state driver's license and proof of auto insurance.

Applicants must be able to successfully perform the essential functions of this position with or without reasonable accommodation.
Sufficient strength, agility and mobility to perform essential functions of position and to supervise program activities.
Salary Description
$24.00-$24.50
