Re-Entry Case Manager (Part Time)

Homeward Alliance is a Fort Collins-based non-profit that operates a continuum of programs providing a range of services, including basic needs assistance, housing-focused case management, behavioral health, and employment support. We also oversee the Murphy Center, the hub of services for people experiencing or at risk of homelessness in Larimer County and are a regional leader in the effort to launch best-practice, data-driven, and collaborative solutions. PROGRAM OVERVIEW

The Re-Entry Program is a program of Homeward Alliance. It provides intensive case management, in coordination with the criminal-justice system, to individuals identified by the Department of Corrections as medium- or high-risk of recidivism. The program is housed at the Murphy Center for Hope (“Murphy Center”), a resource center in Larimer County for people who are homeless or at-risk of experiencing homelessness.

The Re-Entry Case Manager works to empower each participant by helping them identify and achieve attainable goals surrounding employment and education to help them regain stability.

This position reports to the Re-Entry Program Manager.

JOB RESPONSIBILITIES:

Participant (Client) Support and Case Management

Ensure that the following services are being provided within the guidelines laid out according to the Work and Gain Education and Employment Skills (WAGEES) Grant expectations. Assist clients returning from prison in reintegrating successfully and sustainably into the community by:

  • Connecting clients with their communities and families
  • Providing clients with employment preparation, placement and retention, work clothing and tool assistance
  • Goal setting and accountability
  • Identification acquisition
  • Vocational Training
  • Transportation Assistance
  • Gang Disengagement
  • Medical Benefits Acquisition
  • Family Reunification / Parenting
  • Education Assistance (fee for classes and books) and more
  • Data Entry, Walk-Ins, Employer Engagement
  • Data must be entered into the WAGEES “CaseMGR” data system, and Department of Corrections (DOC) CWISE system. Ensure goals and outcomes are being met and data is being inputted in real time.
  • Attend bi-weekly Re-Entry staffing meetings and provide the Re-Entry Program Manager with up-to-date information on current caseload.
  • Search for new Fair Chance/felon-friendly employers to create relationships and an understanding of the challenges of our clients, with the goal of obtaining employment opportunities.
  • Meet with walk-in participants to gauge potential engagement, needs and eligibility.
  • Monitor and manage the referral/waitlist process, and outreach to participants as needed.
  • Attend quarterly WAGEES meetings in Denver to provide the Latino Coalition for Community Leadership (LCCL) with program updates and to learn from other organizations operating under the same grant.
  • Work on building relationships with other community agencies that support the same population: DOC, Parole Office, community correction facilities, and other non-profits.
  • Other duties as assigned.

SKILLS AND QUALIFICATIONS

  • Motivation to work with people who have diverse identities and backgrounds, have significant life experience with the criminal justice system, and are often institutionalized (more specifically, medium to high-risk people exiting the system who are at risk of recidivism).
  • Must be willing to work with people who have been charged with sex offenses.
  • Work experience and/or bachelor’s degree in related field (social work, career counseling, case management, etc.)
  • Strong computer skills, including demonstrated success with Microsoft Office Suite; strong skills and comfort with internet applications.
  • Alignment with Homeward Alliance’s values and commitment to practicing efforts which promote diversity, equity, and inclusivity, as well as Housing First philosophies.
  • Ability to work both independently, collaboratively, and with minimum supervision in a fast-paced, high-stress environment.
  • Detail-oriented and organized.
  • We encourage candidates with past lived experience (homelessness, substance use recovery, criminal justice involvement, a criminal record) and WAGEES program experience to apply.
  • Experience working with vulnerable/marginalized populations preferred.
  • Bilingual/Spanish speaking preferred.

ESSENTIAL JOB FUNCTIONS

  • Ability to be present in-person for work every day. This is not a remote/virtual job, though some flexibility to work from home may be allowed after an initial 90-day probationary period.
  • Professionally engage diverse stakeholders, colleagues, and clients.
  • Able to remain in a stationary position 50% of the workday, using a computer and associated technology.
  • Must be able to attend community meetings virtually and/ or in-person, as well as visit partner agencies in-person via own transportation or possess a driving record and valid driver’s license (to access agency vehicles).
  • Must be able to pass a background check, legally work in the United States, and maintain confidentiality of protected health information.
  • Must be able to pass the WAGEES background check requirements.

SALARY, BENEFITS AND SCHEDULE:

This is a part-time, hourly position for 25 hours per week. The position requires some flexibility and can include some evening and weekend hours. Shifts will typically be 8 am-1 pm Monday-Friday, primarily at the Murphy Center.

Salary: $23.10 / per hour. Additional $0.50 / per hour pay for bilingual employees.
