Position Overview and Job Duties

Must Have the Following:

Possession of a certificate, license, and/or degree from an accredited institution in social work, public health administration, business administration, public administration, organizational leadership, or an equivalent field of study or solid experience.

A minimum of two years of work experience as a Rapid Rehousing Case Manager is required.

Rapid Rehousing Case Manager

The role of a Rapid Rehousing Case Manager is to assist individuals and families experiencing homelessness in quickly obtaining and maintaining stable housing. Their primary responsibility is to provide case management services to clients, helping them navigate the process of finding suitable housing options, accessing financial assistance, and addressing any barriers they may face in securing and maintaining housing stability.

Assessment:

• Conduct thorough assessments of clients' housing situations, including their current living conditions, financial situation, and any barriers to housing stability.

Housing Search Assistance:

• Assisting clients in identifying available housing options that meet their needs and preferences, including affordable rentals, transitional housing, or other supportive housing programs.

Financial Assistance:

• Connecting clients with financial resources such as rental assistance, security deposits, or utility assistance to help them obtain and maintain housing.

Case Planning:

• Developing individualized case plans in collaboration with clients, setting goals for obtaining and maintaining housing stability, and outlining steps to achieve those goals.

Coordination:

• Collaborate with other service providers, community organizations, and government agencies to coordinate comprehensive support services for clients, including healthcare, employment assistance, and social services.

Referrals and Advocacy:

• Making referrals to community resources and support services, such as mental health treatment, substance abuse counseling, employment services, or educational programs. Advocating on behalf of clients to address systemic barriers to housing stability.

Supportive Services:

• Providing ongoing support and guidance to clients as they navigate the challenges of securing and maintaining housing, including budgeting assistance, landlord mediation, and crisis intervention.

Documentation and Reporting:

• Maintaining accurate records of client interactions, progress, and outcomes, and submitting required documentation and reports to funding agencies or regulatory bodies.

Collaboration:

• Collaborating with other service providers, government agencies, landlords, and community organizations to coordinate services and resources for clients.

Crisis Intervention:

• Providing immediate assistance and support to clients facing crises related to housing instability, such as eviction threats or unsafe living conditions.

Follow-Up and Aftercare:

• Conducting follow-up assessments and providing aftercare support to clients after they have secured stable housing, to ensure they maintain housing stability and address any emerging needs or challenges.

The Rapid Rehousing Case Manager role demands a blend of leadership, housing expertise, case management skills, and a deep understanding of the complex challenges individuals or families face in securing and maintaining stable housing. Also, play a crucial role in helping individuals and families experiencing homelessness regain housing stability and work towards long-term self-sufficiency. Must possess strong interpersonal skills, empathy, resourcefulness, and a deep understanding of the complex factors contributing to homelessness and housing insecurity.

Qualifications and Competencies

Must have experience in Public Housing Sector; Permanent Supportive Housing (i.e. Emergency Housing, Rapid Re-Housing, and ReEntry Housing Program)

Proficient in Case Management

Time management

• Excels at time management and organization.
• Demonstrates competence in task prioritization and resourcefulness.
• Capable of balancing multiple demands concurrently.
• Supports staff with time efficiency to meet tight deadlines.

Technical skills

• Proficient with Microsoft Word, Outlook, and PowerPoint; has knowledge of spreadsheets and advanced Excel skills.
• Experienced in using Zoom, Teams, Skype, and other virtual meeting platforms.
• Willing to learn and use new software and databases as needed, including Homeless Management Information System (HMIS).

Compensation

• Salary is competitive (inclusive of fringe benefits)
