JOB DESCRIPTION:

The Peer Outreach/EASE Specialist implements the HIV, Syphilis, and Hepatitis C outreach, client recruitment and retention and social media for the EASE program and its activities. This position will have a heavy focus on MSM of color, YMSM, and Trans women living with or at risk for HIV with a dual identified substance abuse history. This position will coordinate substance abuse related services between providers of HIV, housing, mental health, and other supportive services, in the region, in order to enhance prevention services for people living with HIV and AIDS and people at risk for HIV.

MAJOR DUTIES AND RESPONSIBILITIES:

The Peer Outreach/EASE Specialist will work under the supervision and administrative guidance of the VP of Operations while carrying out the following responsibilities:

  • Work from the EASE facility one full day a week in order to focus on client retention, planning and engaging with the LGBTQ group.
  • Work the required days from the EASE facility focusing on delineated projects
  • Engage in outreach by finding and connecting with potential clients in the community
  • Bring clients and potential clients to EASE, familiarizing them with the EASE facility, staff and resources.
  • Provide face to face and online education and information on HIV, prevention, safer-sex techniques, HIV testing and ongoing education to clients and their families as well as other organizations, families and individuals as requested
  • Assist in development and maintenance of the EASE social media presence with multiple groups, posts and ways to boost and promote the EASE program
  • Follow-up on inquiries from social media and any other source, connecting individuals to services
  • Work closely with other members of the RAIN outreach team to plan routine HIV/HCV testing at partner agencies, in the community and with individuals as needed
  • Build and maintain trusting, effective and empowering relationships with the clients/enrolled peers ensuring all quality assurance measures are met
  • Work closely with and proactively to connect clients to community health organizations, medical service providers and other services
  • Track Intervention and Outreach with verification documents
  • Track and follow-up with clients who have fallen out of care and work to reconnect them with services
  • Document all contacts with participants or others pertaining to the participants in approved format within the time deadlines

MINOR DUTIES AND RESPONSIBILITIES

  • Working knowledge of and adherence to HIPAA regulations, attend all required trainings and maintain confidentiality of client's records, situations and information
  • Meet all programmatic and third party deadlines; maintaining and providing all billing records, reports, daily and monthly work records, time sheets, surveys, requests and tracked mileage when due and in approved format
  • Work effectively with other community service providers to ensure optimal quality of services offered to participants and to develop new resources to meet the needs of the participant
  • Understand, utilize and maintain core components of EASE Interventions
  • Participate in continuous quality improvement for the department and the agency
  • Be a mentor for the participants

MINOR DUTIES AND RESPONSIBILITIES:

  • Participate in training, conferences, supervision, agency and inter-disciplinary meetings and required agency functions
  • Must be willing to be trained/certified as a Certified Application Counselor for the ACA

QUALIFICATIONS:

  • Two years' work experience in HIV preferred
  • Must have a valid driver's license, reliable transportation and auto insurance coverage
  • Living with HIV and successfully virally suppressed
  • Knowledge of HIV and other STI's
  • Demonstrate leadership skills
  • Ability to comfortably and professionally build, maintain and navigate working relationships across the agency spectrum
  • Public speaking, training and strong interpersonal communication skills with an ability to communicate effectively verbally and in writing and willingness to train others
  • Self-starter able to work with limited supervision with skills in creative thinking and problem solving
  • Ability to demonstrate cultural competency and understanding of the structural barriers impacting health outcomes
  • Knowledge of and expected willingness to remain current on LGBTQ+ cultural issues

KEY COMPENTENCIES:

  • Comfort in generally using a computer with proficiency in Word and Excel and aptitude and willingness to learn other job specific programs
  • Passing score on HIPAA regulations training, understanding of confidentiality and expected willingness to maintain these standards at all times
  • Ability to recognize and maintain personal and professional boundaries practice regular self-care
