As an expert clinician, is responsible for the coordination of all systems/services to ensure an organized, multidisciplinary approach to the delivery of care to the Radiology patients. As the coordinator of care to the outpatient population within Radiology, work directly with the Radiology physicians to assure seamless patient flow including attending clinic.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Current licensure as a Registered Nurse in state of practice.
2. Obtain certification in Basic Life Support within 30 days of hire date.
EXPERIENCE:
1. Three (3) years of nursing experience.
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelor of Science in Nursing and/or Advanced degree such as MSN, MPH, MPA.

1. In collaboration with other systems/services, develops and implements a patient based plan of care. Provides coordinated and comprehensive patient care.
2. Assures the delivery of comprehensive services through the coordination of direct or indirect provision of expert patient care for the service.
3. Acts as a liaison between patients, physicians and the hospital in transitioning the patient throughout the healthcare system including outpatient and inpatient environments.
4. Attends clinic with the interventional physicians to obtain necessary information for communication with other departments/agencies regarding patient statues and healthcare needs.
5. Coordinates Radiology exams and/or procedures with providers and anesthesia. Collaborates with radiologist and/or surgeon on the approval of procedures.
6. Provides patient instructions for preparation of exams/procedures. Conducts pre procedure patient education and sets up further appointments if necessary.
7. Triages incoming patient/referring physician calls. Performs proactive outgoing telephone triage by routinely contacting hi-risk patients and caregivers to check on health status and service needs. Collaborates with triage call room and other departments to coordinate the correct placement of patients into proper clinics.
8. Coordinates long-term follow-up of patients with their local/referring physicians as needed/directed.
9. Assists the provider/division/multidisciplinary team in quality improvement activities.
10. Maintains effective communication with Department Leadership, Medical staff, patients, clerical staff and other departments as necessary to assure identification of problems and provide timely problem resolution in support of the hospital's mission of quality in patient care.
11. Assesses patient problems including type and level of care needed through phone and face to face triage to maintain efficient clinic flow and effective patient care. Assess patient/family satisfaction issues and takes appropriate action to resolve. Assists Department Leadership in resolving patient complaints. Promotes customer satisfaction through response to customer perceptions of service in a professional and constructive manner.
12. Ensures equipment is in good service and operated safely and properly by staff.
13. Assists in facilitating monthly clinical and clerical staff meetings.
14. Participates in Performance Improvement activities as appropriate. Including providing feedback as requested for revaluating employee performance on a yearly basis.
15. Assists with insurance pre-authorization, coordination of ancillary testing, scheduling, and referrals. Contacts medical insurance companies to provide comprehensive medical information in order to obtain authorization. Liaison with outpatient managed care specialists and inpatient precertification personnel.
16. Ensures all necessary testing and scheduling is complete prior to both outpatient and inpatient procedures.
17. Collaborates with financial counselors and social services to assure completion of financial assessments. Assists in making appropriate referrals for patients with limited/no resources. Coordinates care with BCCSP to find programs available for financial assistance.
18. Collaborates with Social Services regarding outlined plan of care and facilitates assistance for traveling to and from appointments. Assists with referrals, problem solving or overcoming obstacles to enable patient to achieve optimal level of breast care services.. Collaborates with plastic surgeons in applying for grants for education and charity care for patients seeking reconstruction options.
19. Assists with completion of healthcare related paperwork (i.e. LOA, FMLA, return to work/school, etc.). Assists in revising order sets, documentation templates, consent forms, clinical guidelines, etc. Compiles reference materials, summarize data and prepare reports, and draft manuals and materials.
20. Contributes positively to the teamwork of the department by assisting other staff, contributing ideas and helping to resolve problems. Serves as role model and promotes within the staff effective communication and conflict resolution skills, reflecting quality service attitudes and keeping consumer satisfaction in mind.
