Quality & Patient Safety Specialist
Full-Time

Overview:
The Quality & Patient Safety Specialist facilitates and participates in the implementation of Hospital for Special Surgery's Quality and Patient Safety Improvement Program in conjunction with the Quality & Accreditation Department leadership. The Specialist supports organizational quality and Patient Safety priorities by identification, development, implementation and assessment of clinical quality and Patient Safety improvement initiatives. The specialist supports HRO initiatives including the ongoing readiness program and tracer activities in compliance with accreditation and regulatory agencies. The Specialist acts as a subject matter liaison to hospital departments/service lines to provide guidance on performance improvement activities through the identification of opportunities for improvement, seeking solutions, and establish monitoring and evaluating strategies. Works in collaboration with all staff including administration, physicians/clinicians, and allied health to support the quality, Patient Safety, and performance improvement process. Other responsibilities may include facilitating data collections to include coordinating publicly reported quality measures, working with federal payment linked quality programs or event reporting to the New York State. Supports the physician quality program. The Quality and Safety Specialist also completes other necessary duties as assigned by the Quality & Accreditation Department leadership.

Responsibilities:
  • Exercises Leadership - Develops knowledge
    • Develops and maintains knowledge of federal and state regulatory requirements related to departmental data, accreditation and Patient Safety, and public reporting programs.
  • Achieves Professional & Business Objectives - Coordination and facilitation.
    • Maintains independent membership and/or ability to facilitate departmental and hospital level Teams. Coordinates and facilities activities associated with workgroups/committees such as meeting agendas, minutes, reports to ensure accurate information and follow-up is completed.
  • Develops/Manages Processes & Systems - Quality and Performance Improvement
    • Applies models for improvement and methodologies and the appropriate change management concepts to achieve successful project outcomes. Develops SMMRT aims and/or family of measures, i.e. operational definitions and sources, for ongoing quality assessment and performance improvement initiatives. Utilizes quality improvement tools to visualize a process, gather and organize information, understand variations and relationships, and to evaluate the output to successfully redesign initiatives toward improved performance. Translates evidence-based knowledge into practice to improve outcomes. Creates, updates and accurately interprets data in a timely manner.
  • Achieves Professional & Business Objectives - Prepares data
    • Conducts ongoing medical record review, analyzes data and compiles reports as required. Prepares and presents data in appropriate format and reviews/validates for accuracy for public reporting and committee deadlines. Prepares RCA documents (i.e. fishbone, RCA... ), performs associated literature searches and ensures throughput of information.
    • Supports the design and implementation of an improvement plan. Establishes and anticipates deadlines associated with routine functions. Independently follows open items through to completion.
  • Achieves Professional & Business Objectives - Recognizes adverse events
    • Understands adverse event definitions as defined by HSS and other regulatory agencies that require further analysis and/or reporting to NYS DOH. Maintains knowledge regarding RCA process. Participates in risk reduction strategies to ensure ongoing assessment of Patient Safety processes and practices in compliance with local, state, federal and regulatory agencies. Evaluates ICD coded complications, grievances, and occurrence reporting referrals for safety or care delivery concerns and succinctly describes concern to relevant stakeholders or peer review committee.
  • Achieves Professional & Business Objectives - Effectively communicates
    • Effectively and efficiently communicates via verbal, written, in person, or remotely with various departments to disseminate and receive information as requested. Provides information and presentations succinctly.
  • Maintains Documentation & Manages Information - Documents review and data abstraction.
    • Conducts document review for Quality Improvement and Patient Safety purposes. Maintains documentation/record, as appropriate, related to the review indicators in accordance with Departmental and Committee directives. Accurately reviews and abstract data according to data specifications and/or supports reports aligned with data definitions for internal and external reporting purposes. Able to analyze data, identify trends and present appropriately to stakeholders.
  • Upholds Standards/Expectations of HSS Staff- Time Management
    • Manages time appropriately to meet project, department, and hospital needs, and if conflicting priorities are present, escalates to supervisor.
  • Upholds Standards/Expectations of HSS Staff- Maintains and develops competence
    • Maintains and develops competence through required in-service and continuing educational programs. Completes, on a timely basis, all staff/manager organization-wide annual in-service modules. Collaborates with peers, physicians, and other members of the team as appropriate.
  • Upholds HSS Professional/Clinical Standards - Promotes a professional atmosphere
    • Promotes a professional atmosphere through courteous communication, cooperation, and respect for patients, visitors and members of the health care team.

Qualifications:
  • A bachelor’s degree required in healthcare associated field, nursing or other related clinical disciplines.
    • A master’s degree preferred in healthcare associated field, nursing, PA or business
  • Certification/Licensure Requirements
    • CPHQ, CPPS
  • 1 - 3 years of experience
  • Experience with exposure to quality assurance/quality management/performance improvement and patient Working knowledge of improvement science tools and methods and basic skills with data management
Other Requirements: #LI-Onsite Pay Range - Minimum: USD $115,000.00/Yr. Pay Range - Maximum: USD $130,000.00/Yr.
