Constellation Quality Health, formerly CCME, is a non-profit health care quality organization and QIO-like Entity certified by the Centers for Medicare & Medicaid Services (CMS) founded by physicians in 1983. Headquartered in North Carolina's Research Triangle, we offer an array of Quality Improvement, clinical review, audit, technical, and consulting services and solutions to improve care delivery, system performance, and patient outcomes.
What You'll do:
CMS has enlisted the Quality Improvement Organizations (QIOs) to assess and support the education and improvement of infection prevention practices in nursing homes. We are seeking Quality Improvement specialists that are registered nurses to travel to nursing homes that have been identified by CMS with active COVID-19 outbreaks in South Carolina. Additionally, Quality Improvement specialists will be assigned tasks that support programs, projects and services lines, offering subject matter expertise in Quality Improvement.
We expect you to:
- Conduct nursing home site visits using CDC Infection Control Assessment for Long Term Care Facilities.
- Identify source of transmission and spread.
- Assess root cause of the outbreak and suggest strategies for improvement.
- Coordinate services and successfully manage multiple projects with limited oversight.
- Maintain knowledge of industry standards, trends, and opportunities as well as organizational strategies and business lines.
- Follow documentation and analytic standards in accordance with organizational expectations and branding.
- Coordinate consulting service needs with a focus on customer service standards.
- Provide advice or suggestions for improvement in accordance with objectives, standards of practice, and regulatory guidelines.
- Present and explain findings to appropriate executives, front line leaders, and supervisors as appropriate, for plan development.
- Evaluate the situation periodically, adjusting as needed to achieve program goals. Provide routine communication and reports, as needed, in accordance with organizational standards.
- Drive customer service through positive, empathetic and professional approach.
- Assist with post acute care consulting service line development.
- Travel reimbursed at prevailing Federal Per Diem rate.
The positions work remotely; however, only candidates located in South Carolina will be considered.
Requirements for Employment:
- Active SC RN license in good standing
- Experience as clinical director within skilled nursing facility. Director of Nursing or regional clinical director experience preferred. SMQT certification will be considered in lieu of director experience.
- Recent experience in infection prevention (3 years or greater, with current knowledge of CDC and CMS guidelines for reducing transmission of COVID)
- CIC certification preferred but not required
- Proficiency in MS Office applications (Word, Outlook, Teams)
- Proficient in Quality Improvement methodologies such as PDSA and QAPI process; and effective time management and organizational skills are required
- Must be able to work independently as well as with a professional team
