The Quality Improvement Specialist is responsible for providing support to the organizations Quality Improvement plan through the implementation of targeted Quality Improvement projects and performance improvement activities.
Schedule is Monday – Friday, working from 8:00am – 5:00pm.
Compensation:
$19.76 - $20.75 an hour
Essential Duties and Responsibilities
- Demonstrates problem solving skills and effective communication, both verbally and in writing.
- Works with health plans through the QI Project Manager (Panel Management) as required/needed to review and address gaps in incentive goals.
- Coordinates implementation of activities for incentive deliverables including calling patients, sending letters, scheduling appointments, etc.
- Works collaboratively with other Quality Improvement staff including QI Project Managers and QI Coaches as well as operational management to implement projects that enhance patient care.
- Works collaboratively with other QI staff and operational management to promote Quality and safety projects.
- Collaborates with local site staff to provide needed training and competency assessments for specific to quality and safety activities.
- Supports the Quality Improvement department in the auditing of workflows as it relates to the documentation of quality indicators.
- Collaborates and supports QI coach teams with trends, care gap reports, and other data in relation to quality metrics.
- Generates accurate and timely QI data.
- Enters data timely and accurately, into established databases, spreadsheets and other electronic medium as needed.
- Completes regular analysis of QI data and relays findings to QI Project Manager.
- Submits and presents reports to QI team about the progress in metrics across the Quality Improvement plan and health plan incentive programs.
- Participates as a preceptor or trainer during general staff training presentations for Quality Improvement activities as directed by QI Project Manager.
- Proactively assumes responsibility for continuous improvement activities.
- Willingness to explore and implement innovative quality programs/projects.
- Data entry, gathering, and analysis for QI grants, projects, and programs.
- Demonstrates understanding of the mission, vision and goals of the health center.
- Other duties and projects as assigned.
Physical Demands
- Lift up to 30 pounds occasionally and push up to 100 pounds (on wheels) on rare occasions.
- Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment
The noise level in the work environment is usually quiet to moderate.
Minimum Qualifications:
- Proficient with MS Office – word, excel and power point.
- Knowledge of Electronic Health Records (EHR), immunization registry, Quality Improvement concepts and performance measurements.
- Follow guidelines from QI Manager and accurately executes the steps for process improvement.
- Relate in a professional manner and handle confidential information.
- Successfully navigate paper based and electronic health records.
- Connect and build strong professional rapport.
- Highly proficient in medical terminology.
- Excellent communication, written and verbal, and strong comprehension skills.
- Valid drivers’ license, vehicle insurance and acceptable driving record.
- Ability to meet deadlines, prioritize tasks and interact effectively with various levels of staff and departments.
- Ability to collect, correlate and analyze data.
- English fluency required.
Education/Experience Required:
- High school diploma or equivalent.
- Associates Degree or Medical Assisting Certificate from an accredited program.
- Two (2) years’ experience in health care required, in the area of Quality Improvement, preferred.
