Community Bridges is looking for a full-time
Quality Improvement Directorin Concord, NH!

We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.


The Quality Improvement Director is responsible for assuring that Community Bridges and the service provider agencies comply with applicable regulations, and provide supports and services that protect and respect individual rights. The Quality Improvement Director is responsible for statistical analysis, and reporting on the agency and state quality assurance indicators as well as Designated Area agency reports (DADDS). The Director will be the second line of oversight of Provider Agency supports and services. The Case Management teams provide the first line of oversight. The Director may initiate or coordinate Quality Improvement audits, record reviews or make recommendations for Projects required to correct/improve internal and regional external processes and practices.


Qualifications:

Education: Bachelor's Degree, preferred

Experience: Five (5) years' experience in the human service or related field with persons with disabilities desirable. A high level of analysis and attention to detail is required. The ability to coordinate multiple tasks and set priorities and meet deadlines is essential.


Compensation and Benefits

There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Specific Duties and Responsibilities:

  • Provider Agency Oversight - works collaboratively with provider agencies to provide support, resources and training as needed. Provides data tracking, summary and analysis for Semi-Annual Provider Agency dashboards and meetings. Processes waivers. Acts as the point person for the DADDS functions in our contract with DHHS.
  • Area Agency Oversight -works collaboratively with agency staff to provide support, resources and training as needed. Supports staff with rights violations, Incident Reports, complaints, Sentinel Event and Mortality Reporting.
  • Quality Improvement Activities - Initiates and coordinates process improvement projects as assigned. These may be specific to a department or to the agency as a whole. Biannual medication reports to BDS
  • Liaison to BDS - Acts as liaison to the DHHS staff including Facilities and Licensing Director, and Complaint Investigators. Uploads documentation to a secure site at the request of BDS or as necessitated by process.
  • Training - Trains provider agency and area agency staff as needed. Trainings may include but not be limited to: Incident Reports, Complaints, Medication Administration Reporting and Rights, Abuse, Neglect and Exploitation.
  • Quality Improvement Committee - Facilitates the Community Bridges Quality Improvement committee, develops meeting calendar and agenda and facilitates meetings.
  • Agency Representation on State and Regional Committees - Represents the agency at the CSNI Statewide Committee. and other meetings as assigned
  • Stays Abreast of Knowledge and Skills in the Theory and Practice of Human Services Administration - Attends training opportunities and conferences. Reads material as recommended and as found in professional literature. Participates in professional trade organization(s) to discuss current issues in human services.
  • Other - Other duties as assigned.


Provider Agency Oversight Tasks:

  • 1201 Medication Reports - Coordinates the process of receiving, reviewing and analyzing the provider agency 1201 reports. Creates a summary report 1201C of all provider agencies medication administration reports and submits to the BDS Medication Administration Committee. This is done in February and August. Attends the medication committee two times a year to discuss our regional reports.
  • Provider Agency Interagency Collaboration - Facilitates the Interagency Advisory Council, a collaborative forum for all provider agencies. Attends and records the meetings. Insures that any updated documentation distributed at the meeting is distributed to provider agency representatives that aren't in attendance
  • Certification/Licensing Reports - Receives reports from the Health Facility Licensing Office and follows up with Provider Agencies on Corrective Action Plans and completion. Tracks Data. Follows up with Area Agency Case Management team on Corrective Actions for deficiencies related to Service Coordination.
  • Incident Reports - receives and tracks Incident Reports. Assesses for completeness and follow-up. Insures revised forms are received in a timely manner. Evaluates incidents to determine if any present a violation of clients rights and takes action as required.
  • Sentinel Event and Mortality Reporting - as they occur, receives reports, review for quality and submits into NH Leads.
  • Employment Data Monitoring - Sends out email reminders to Provider Agencies to update their data on a quarterly basis. Distributes BDS Employment Reports to Provider Agencies. Follows up with Provider Agency management when data is not being updated according to Guidelines.
  • Waivers - Receivesfrom provider agencies to waive a regulation. Reviews and makes recommendation to Executive Director/Designee. Submits to BDS upon approval.
  • ITS - Site visit Forms - receives and reviews for areas of concern.


Area Agency Oversight Tasks:

  • Chart Audits - Periodically, according to the Quality Assurance Calendar, coordinates chart audits for various agency departments to review records for quality, completeness and compliance with regulations.
  • Complaint Investigations - Tracks incidents of consumer rights violations, service dissatisfaction, and abuse/neglect in community based services by overseeing formal complaint investigations and working closely with all invested parties to identify and ameliorate situations of rights violations and poor quality services and supports. Provides agency administration and the State with statistical data, trends analysis and recommendations for systemic improvements. Coordinates investigations and assure the timeliness of investigations in accordance with the He-M regulations.
  • Protection of Individual Rights - Assures that staff, managers and provider agencies protect and demonstrate respect for each person's legal, civil, and human rights. Provides staff and provider agency trainings, reviews client records and analyzes data to assure compliance with regulations regarding rights, and provides technical assistance to individuals, families, and guardians regarding rights upon request. Participates in and assists with various activities for the Human Rights Committee and the Internal Review Committee, including taking minutes, preparing and processing mailings, and tracking data and scheduling meetings.
  • Quality Improvement Reports - Provides agency, provider agencies, CSNI and the Bureau of Developmental Services with reports regarding quality of services and supports. Utilizing many sources of data, reports to be generated on a regular basis include, but are not limited to: Semi-annual Individual Provider Agency Reports, Chart Review Summary and Recommendation Report, and Semi-Annual Complaint Investigation Summary. Receives NCI consumer survey data and puts in a useable format for reporting.
  • Coordinates Agency Quality Assurance Activities - Oversees the QA Calendar and coordinates Quality Assurance Activities including chart audits, surveys, data collection, analysis and reporting, in compliance with He-M Regulations and Re-designation requirements.

9.Must possess and maintain a valid NH license and safe driver's record in accordance with Community

v. Bridges standards.

10.This position is an in-office position with negotiated time to work from home


Technical Requirements:

  • Demonstrate computer competency, comfort and capability with Microsoft office software at an intermediate level of Excel, Word, Access and Outlook.
  • Experience working in a network environment.


Travel:

Ability to travel (access to personal vehicle during all scheduled work hours). Must possess and maintain a NH license and an acceptable driver's record in accordance with Community Bridges standards.
