DWGAS (“DW”) is about doctors working to improve healthcare by giving back and serving the communities where they practice. DW helps to organize independent primary care doctors (“PCPs”) into accountable care organizations, who take responsibility for the total cost and quality of care for their patients, and in exchange they can receive a portion of the savings they achieve. Furthermore, DW provides the underlying infrastructure, operations, and technology to enable PCPs to deliver better quality care at a lower cost.

 

DWGAS is seeking a Quality Improvement (QI) Coordinator to join our team! This individual will work with the Quality and Operations team to ensure that all internal and external Quality Improvement opportunities are identified, documented, and tracked, areas of improvement are communicated, resolution plans are coordinated with the practices, and interventions are updated as needed with the highest performance standards. The goal is to improve RAF, HCC, TCOC, Quality, and Stars Measures, leading to better overall patient care and health outcomes by supporting the medical group’s physicians and practices and maintaining the highest level of Quality Improvement performance status across all lines of business. The QIC ensures QI performance reports are delivered, and interpreted, intervention opportunities are offered and communicated, workflow adjustments are suggested and trained, and collaboration and relationships are well established among our medical group provider network.

 

Essential Duties and Responsibilities:

 

Coordinates with Providers and staff to ensure program related activity are completed and supports staff for the administration and improvement of the quality program.

  • Provides support in managing program priorities, resource constraints, program issues, and mitigating barriers to plan the work activity for the program and ensure timelines and deliverables are understood and achieved by staff.

  • Serves as subject matter expert on the quality program and any platforms deployed and used as program tools.

  • Supports the development of business requirements or statements of work necessary to support and reach quality goals.

  • Ensures program documentation and reporting (e.g., program requirements, guide, workflows, decision documents, etc.) are complete and meet applicable audit and compliance requirements.

  • Evaluates the program for areas of improvement (e.g., execution of quality measures, display of quality measures in program platforms, and/or adding/improving functions within the program or program platforms for ease of user experience)

  • Schedules and conducts office site visits, check medical records and other clinical reviews as required in accordance with established standards.

Collaborates with cross-departmental entities and external partners to administer mutually beneficial quality programs.

  • Support and develops strong business partnerships with key departments and areas.

  • Support the organization and external partners to continually improve quality performance.

  • Obtains guidance from leadership

  • Serves as a quality coordinator in external stakeholder (e.g., network providers, contracted vendors, and hospitals, etc.) for meetings and presentations.

  • Commit to support functional areas to keep the quality subject matters successfully coordinated.

  • Provide ongoing performance and action plan updates to appropriate internal stakeholders.

  • Ability to identify and suggest monthly agenda's and facilitate meaningful discussion to monitor, evaluate and improve the quality care gaps and practice workflow.

  • Ensure that various quality indicators are discussed and properly analyzed and that recommendations resulting from indicator monitoring are reported through the Quality Improvement leadership.

Providing strategic insight into quality affairs.

  • Gathers data from historical and current performance in the quality program and identify trends, issues, developments, and areas for opportunity.

  • Anticipates potential impacts to current and future Quality Improvement opportunities.

  • Once solutions or opportunities are identified, provide a recommendation, and execute the selected recommendation after leadership’s approval.

  • Serve as Quality Project Coordinator for additional and or new Quality Improvement Activities and projects.

Performs all other miscellaneous responsibilities and duties as assigned or directed.

  • Other duties as assigned related to ensuring that various Quality Improvement Standards are met, including coordination of HEDIS and other mandated activities.

 

 

Qualifications and Requirements:

 

  • Bachelor's degree and three years of healthcare or medical group related work

  • experience; or equivalent combination of education and related work experience.

  • 2 years of quality, care coordination or Medical Assistant experience.

  • Strong working knowledge of care and Quality Improvement.

  • Effective written and verbal communication.

  • Knowledge of health care industry trends.

  • Intermediate working knowledge of Microsoft Office applications: including but not limited to Word, Excel, Outlook, and Power Point

  • Must reside in Honolulu, Hawaii.

  • Must be able to work in Hybrid Work Environment. Travel and office sites to support projects are required.

  • Strong training experience is highly preferred.

  • Knowledge of health care terminology is highly preferred.

  • Tagalog or Ilocano speaking is preferred but not required.
