In addition to behavioral health care, Metrocare provides primary care centers for adults and children, services for veterans and their families, accessible pharmacies, housing, and supportive social services. Alongside clinical care, researchers and teachers from Metrocare's Altshuler Center for Education & Research are advancing mental health beyond Dallas County while providing critical workforce to the state.

The Qualified Mental Health Professional working in the TCOOMMI program, will provide intensive, community-based, therapeutic intervention and supervision to adult and or youth on probation. Services also include monthly medication monitoring with psychiatrist. provides psychosocial/rehabilitative service to persons with mental illness. Services may include, but are not limited to, assessment, development and measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention and continuity of care. Provide individual, marriage, family and group counseling. The overall goal of this position is to maximize the consumers potential level of functioning, reduce hospitalization and aid in the successful reintegration of consumers into the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Identify and assist consumers in obtaining entitlements by providing referrals, advocacy and negotiation, as needed.
  • Co-facilitate family education workshops.
  • Provide individual, family, and group rehabilitation sessions. Evaluate progress of counseling session, solicit feedback from consumer(s), and consult with colleagues and team leaders when dealing with unfamiliar/uncomfortable issues.
  • Facilitate inpatient admission upon request of the treatment team. Provide continuity of services throughout inpatient stay. Participate actively in discharge planning. Meet with the consumer within 5 days of discharge into the community.
  • Provides crisis intervention during the work week. Make home and hospital visits, initiates mental illness warrants, work with police and other public servants as a needed to address crisis.
  • Follow-up within 24 hours on assigned consumers who access emergency services. Report status-disposition to treatment team. 100% compliance expected.
  • Oversees and assists assigned consumers medication adherence. Provides med training, arranges transportation to scheduled office based appointments, and prompts needed refills.
  • Follow-up assigned consumers who miss a scheduled clinic appointment. Document attempts to contact consumer. Report repeated unsuccessful efforts to contact the treatment team.
  • Participate in interdisciplinary team staffing. Present psychosocial assessment findings. Encourage consumer to attend staffing. Update team on progress or lack of progress in reaching agreed upon goals. Provide relevant information that might effect treatment.
  • Review service plans every three months or as clinically indicated. 100% compliance expected.
      • Document attempts to involve individual, service providers, and caregivers unless opposed by the individual) in service plan process.
      • Actively assist in obtaining and modifying goals as needed.
  • Develop measurable objectives and goals agreed upon by the consumer.
  • Formulate individual service plan based on assessment findings on all new admission. 100% compliance expected.
  • Complete a psychosocial assessment on each new admission. Upon assessment as clinically indicated. 100% compliance expected.
  • Provide overall service coordination, referral, linkage, and advocacy to a caseload of up to 25 intensive consumers with varying needs. Perform duties in concert with other members of an interdisciplinary treatment team.
  • Performs other duties as assigned.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

  • Bachelors degree in human services with 1 to 3 years experience in MH/MR or related field or Masters degree with 0 to 2 years experience in human services.

MATHEMATICAL SKILLS:

  • Basic math skills required.

REASONING ABILITY:

  • Ability to successfully use an automated clinical record keeping system.
  • Able to work flexible hours.
  • Able to work independently with minimal supervision.
  • Ability to organize and prioritize tasks.
  • Effective written and verbal communication skills.
  • Able to maintain work in 95% compliance of standards at all times.
  • Ability to work as a team member.
  • Skilled in using tact and diplomacy in interacting with staff and individuals.
  • Ability to problem solve, exercise good judgment, and make sound decisions.
  • Ability to effectively manage a large caseload of individuals with varying needs.
  • Ability to organize work flow and develop efficient clinic procedures
  • Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.

COMPUTER SKILLS:

  • Ability to utilize Internet for resources.
  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).

CERTIFICATIONS, LICENSES, REGISTRATIONS:

  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
