Job description

To provide professional Public Health nursing services, supervision of staff, and clinical and administrative oversight that promotes health for residents in the Bloomington, Edina, and Richfield (BER) communities. The primary responsibilities include coordinating immunizations and disease surveillance.

Direct Services (50%)

  • Assessment & Surveillance, Communicable Disease Control
    • Assesses the health and psychosocial needs of individuals and families.
    • Provides Public Health nursing services to individuals and families (i.e., immunizations, flu vaccinations, infectious disease and perinatal Hepatitis B follow-up, etc.).
    • Follows protocols approved by the medical director.
    • Provides tuberculin skin testing and follow-up.
    • Completes health and disease surveillance per MDH protocols.
    • Documents work on time.
    • Completes vaccine reviews in MIIC (MN Immunization Information Connection).
  • Emergency Preparedness & Response
    • Performs Public Health emergency response functions as trained and assigned within the department's emergency response plan.
    • Support the development of emergency response plans that focus on infectious disease response.
  • Communications
    • Responds to inquiries about Public Health services and makes appropriate referrals and links to community resources.
    • Provides science-based and up-to-date information to clientele and community partners.
    • Communicates effectively and builds rapport with people (both employees and the public) from various racial, ethnic, and socioeconomic backgrounds.

Administrative Services (50%)

  • Organizational Competencies
    • Coordinates work plan with Community Health Services Manager; identifies problems and initiates timely, sound responses.
    • Establishes necessary policies and protocols to promote efficient Public Health nursing services.
    • Provides formal supervision and evaluation, as well as regular coaching, development, and orientation for assigned staff.
    • Oversees and maintains accurate and timely documentation of client services via electronic health records.
    • Organizes staffing and clinic schedules.
    • Coordinates Mobile Health Hub vaccination outreach with designated staff and community partners.
    • Ensures that plans and procedures for vaccination clinics are in line with emergency response plans to build staff competencies and support training on those plans.
    • Assists with grant and contract writing and reporting.
  • Accountability & Performance Management
    • Defines the level of expected performance, provides the necessary resources and support, and holds staff accountable for achieving performance goals and completing responsibilities.
    • Coordinates project development, implementation, and management.
    • Creates and nurtures a culture of continuous improvement, community engagement, and equitable and inclusive service.
    • Plans, organizes, implements, directs, and evaluates projects that support the Division's work plan.
    • Oversees annual MNVFC compliant site visits at local clinics.
  • Community Partnership Development
    • Maintains effective public relations and communications with community agencies, schools, organizations, and healthcare facilities.
  • Equity
    • Supports and advances organizational development efforts such as developing a high-performing organization, employee engagement, workforce development, inclusion and equity, performance measurement efforts, and development of a culture focused on excellent customer service.

  • Other
    • Performs other related duties as apparent or assigned.
    • Ability to work flexible hours occasional evenings and weekends.

KNOWLEDGE, SKILLS, & ABILITIES REQUIRED

  • Ability to use logic and reasoning to effectively identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
    • Knowledge of modern-day Public Health best practices and evidence-based models of service delivery.
  • Skill in monitoring/assessing the performance of self, other individuals, or the organization to make improvements or take corrective action.
  • Ability to plan, organize, implement, direct, and evaluate programs that support the Division's work plan.
  • Ability to effectively mentor, delegate, and/or supervise the work of others.
  • Ability to train others.
  • Ability to work flexible hours, occasionally working evening and weekend hours to attend meetings/events to meet client needs.

MINIMUM QUALIFICATIONS

  • Bachelor's or Master's degree in nursing.
  • Current license as a Registered Nurse in the State of Minnesota.
  • Certification (may be pending) as a Public Health Nurse in the State of Minnesota.
  • Five years of recent experience (i.e., within the past eight calendar years) in Public Health or other clinical nursing environments.
  • Experience providing adult and/or childhood routine immunizations.
  • Experience utilizing electronic health records and/or other charting software.
  • Experience working effectively across cultures.
  • Have either a valid driver's license, car insurance, and dependable car or access to reliable transportation required to perform job duties.


DESIRABLE QUALIFICATIONS

  • Advanced degree or certification related to nursing, Public Health, or health informatics.
  • Experience supervising employees or volunteers.
  • Experience working with children, adults, and/or families who are at high risk for poor health outcomes (i.e., low socio-economic status or mental health issues).
  • Experience working with diverse employees, cultures, and interpreters.
  • Experience working on grant projects and budgets.
  • Experience developing clinical protocols or policies.
  • Experience managing lab or vaccine inventory.
  • Experience utilizing the MN Immunization Information Connection (MIIC) state tool.
  • Experience working with interdisciplinary teams.
  • Experience monitoring temperature control technology for vaccine storage.
  • Experience working with communities on vaccine awareness education.
  • Certification in Basic Life Support CPR.
  • Ability to speak a second language, in addition to English that has a recognized presence in the community. (Spanish, Somali, Vietnamese, Chinese Mandarin, Cambodian, etc.)

SUPERVISION OF OTHERS

This position supervises Public Health Nurses and other staff as assigned.

CONDITIONS OF EMPLOYMENT:

  • Must maintain license as a Registered Nurse in the State of Minnesota and certification as a Public Health Nurse in the State of Minnesota.
  • Must maintain ability to travel to community sites, meetings, or training as needed.


Anticipated Hiring Range: $88,159 - $93,528 Annually

2024 Full Salary Range: $88,159 - $117,530 Annually
