Granite State Independent Living is a statewide nonprofit whose mission is to promote quality of life with independence for people with disabilities. We empower people with disabilities to live on their terms and we do this by offering 17 person-directed programs.

GSIL is seeking a Public Health Nurse to help identify and develop community programs addressing social determinates of the community's health. Working collaboratively with the GSIL team, design a program to improve overall community health & safety and increase access to care.

Services to be completed by September 30, 2024.

GSIL is seeking someone to carry out the following deliverables:

• Develop Public Health programs, review, and evaluate activities and related work to be carried out by GSIL program staff.
• Design and assist with the implementation of community prevention strategies and/or the delivery of evidence-based care and services to address community needs.
• Set local priorities for health- related interventions to provide the greatest benefit to the most people.
• Design and implement health promotion programming such as blood pressure clinics, health assessment (in person or over the phone), diabetes screening, cholesterol testing, immunizations, screenings etc.

Required Qualifications

• Registered Nurse with current New Hampshire License and five years of professional experience in program management.
• Master's degree in Public Health or related field.
• Monitor health trends and identify health risk factors unique to specific communities.
• Significant knowledge of federal and state Public Health programs and services.

Cost of Project to be determined based on expectations of completion.
