POSITION SUMMARY
The Rehabilitation Specialist works as a member of an interdisciplinary team. The Rehabilitation Specialist’s primary duty is to assist persons in developing coping skills and living strategies whereby they can achieve maximum autonomy in their home community. The Rehabilitation Specialist serves as primary contact for a diverse caseload of persons with severely and persistently mentally ill adults aged 18 years and older. within facility based setting.
- The Rehabilitation Specialist’s goal is to increase the person’s capacity for independent community living, through the establishment of person rapport, modeling, teaching and the provision of activities and learning experiences.
- The Rehabilitation Specialist provides support to enhance the person’s ability to survive, stabilize and function in the community
- The Rehabilitation Specialist consistently exercises sound discretion and independent judgment in the performance of the duties of the position.
EDUCATION & EXPERIENCE:
- Bachelor’s Degree in psychology, counseling, social work, vocational rehabilitation, behavioral science field, or related field with at least 1 year experience or meets Qualified Professional standards.
- Internships and practicums may be taken into consideration.
- Experience with chronically mentally ill clients, assertive community treatment, day programs or a similar community support setting is preferred.
