We have an amazing opportunity here at New Bedford Community Health (NBCH) - a fast-paced Federally Qualified Health Center (FQHC), dedicated to providing a variety of primary care and MENTAL HEALTH services, to a diverse group of underserved patient populations in New Bedford, MA. We are seeking an experienced Nurse Practitioner with a strong background in MENTAL HEALTH evaluation and treatment, substance use disorders, medication management and treatment planning.
- Provides outpatient care to patients showing a change or decline in mental status, chronic or urgent medical conditions
- Works as an individual contributor to the patients’ medical care team, with entitlements to specific clinical privileges approved by the Health Center’s Chief Medical Officer and Board of Directors, to provide multidisciplinary patient care
- Completes patients’ history and physical and risk factor assessments; orders therapeutic and diagnostic tests; and attends to consultations and appointments
- Prescribes medications following the Health Center’s policy, formulary and relevant regulations of the State of Massachusetts
- Consults with other physicians on cases that are beyond the scope of qualification of PMHNP
- Records all relevant patient data in the patients’ electronic medical records and case presentations
- Formulates, executes and reviews healthcare education delivered to patients and families for diseases prevention
- Communicates effectively with other health care professionals to ensure comprehensive and effective delivery of patient care
- Consults with the Health Center’s nursing and medical staff for improvement in their work performance, which enhances overall quality service delivery
- Masters of Science (MS) or Doctor of Nursing (DNP)
- Active DEA and Massachusetts Controlled Substance Registration
- Certification/Licensure: Family Nurse Practitioner Board Certified (FNP-BC) or Board Certification in Psychiatric Nursing (PMHNP-BC) through American Academy of Nurses Credentialing Center (ANCC) – strongly preferred
- Current BLS/CPR Certification
- Minimum of three (3) years independent practice experience is required
- Experience in community and public health
- Comfortable with supporting LGBTQ+ population as well as individuals with substance abuse disorders (OBAT)
- Commitment and interest in working with multi-cultural and impoverished populations
- Primary or Secondary language skills in: Portuguese or Spanish (preferred)
