Envision Unlimited is a large non-profit agency committed to providing services for adults with intellectual/developmental disabilities and/or mental illness. Our programs include, but are not limited to, ABA Therapy for Children, Community Day Program, Community Living, Mental Health, Employment and Career Services, and Foster Care.


Purpose:

Member of the interdisciplinary team, providing psycho-social rehabilitation and community support group (skill building) services to persons with severe and persistent mental illness in the Psycho-Social Rehabilitation (PSR) Program. Instructors must be independent in providing recovery based, trauma informed curriculum that includes psychoeducation, symptom management, CBT, DBT, expressive therapies, narrative therapy, social skills, therapeutic recreation, developing independent living skills and other evidence-based practices. The goal of the PSR Program is to improve our clients’ ability to manage their MI symptoms, gain insight, increase self-awareness, decrease social isolation, increase engagement in the community and improve overall independence and functioning. PSR instructors will participate in the implementation of client’s individual treatment plans, behavior management programs, and complete documentation as required by the multidisciplinary team, CARF Accreditation, and funders, including Medicaid standards. PSR instructors must meet established productivity targets as assigned.


Essential Responsibilities include but are not limited to:

· Facilitate PSR groups and community integrated services based on the recovery model.

· PSR instructors provide services that align with client’s individual treatment plans.

· Provide individual intervention/services as needed.

· Document all incidents (behavioral, medical, accidental, etc.) in accordance with agency procedures and practices.

· Demonstrate evidence-based, clinical, and ethical practices.

· Always maintain confidentiality according to the Illinois Mental Health code and Confidentiality Act.

· Ensure all services provided are in accordance with client’s individual treatment plan.

· Independently utilize resources to creatively prepare and submit evidenced-based curriculum for PSR groups in accordance with department expectations and timelines.

· Maintain a clean, neat, and safe group environment for all clients.

· Document services to members as outlined in their treatment plan in individual and/or group formats. Notes must be compliant with agency standards and completed by the end of each day.

OVERALL, JOB PERFORMANCE:

· Perform duties and functions in accordance with all agency policies

· Demonstrate professional conduct and behaviors consistent with agency mission and policies when engaged with staff, clients, external stakeholders and when representing the agency in the community

· Will maintain a clean and neat appearance and follow business casual dress code.

· Required to attend individual supervision with Assistant Director of PSR to review and report on progress, productivity, professional development, and address concerns.

· Responsible for participating in, and completing, required in-service and community training seminars and training at the discretion of the Supervisor and/or Director.

· Maintaining state professional licensing requirements, if applicable.

· Communicate observed client needs, clinical assessment, and significant incidents to the interdisciplinary treatment team.

· Participate in large group client consultation.

· Other duties as assigned

Requirements:

MINIMUM QUALIFICATIONS: Master’s degree in social work, counseling, expressive therapies, or related Human Services field required. One year's experience working in social services strongly preferred. Candidate must be at least 21 years of age, possess a valid driver's license, be willing to drive a 15-passenger van and approved to drive agency vehicles through agency insurance, pass a criminal background check and the TABE. Must have excellent written and oral communication skills to facilitate and document curriculum and recovery-based groups.

Must be able to set and maintain professional boundaries, be able to adapt to changing needs of the program and be able to implement de-escalation and crisis intervention techniques as needed.
