Job Description
Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance use disorder, as well as an array of other related problems. Since 1969, FGC's comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.
FGC's adult and adolescent programming provides a flexible continuum of care to best address the individual needs of each participant. Through integrated and comprehensive services, including continuous assessment, participants are able to access levels of care and specific services to create an individualized treatment approach. Further, FGC provides special and high-risk populations priority access to specialized programming.
****$2,000 SIGN ON BONUS****
1st $1,000.00: after 30 days of employment
2nd $1,000.00: after 6 months of employment
Essential Duties:
  • Promotes a welcoming, positive customer service culture which encourages quality patient outcomes and promotes staff retention.
  • Develop patient treatment plans and conducts continued stay review based on research, clinical.
  • Provide patient intakes, treatment planning, case coordination, staffing, aftercare planning and discharge with individual and group therapy for a full spectrum of MAT referrals.
  • Complete and maintain accurate records or reports regarding patients' histories and progress, services provided, or other required information.
  • Interview patients, review records and confer with other professionals to evaluate individual's mental and physical condition and to determine their suitability for participation in a specific program.
  • Coordinate counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, or social workers.
  • Review and evaluate patient's progress in relation to measurable goals described in treatment and care plans.
  • Modify treatment plans to comply with changes in patient status.
  • Intervene as an advocate for patients to resolve emergency problems in crisis situations.
  • Counsel patients individually or in group sessions, to assist in overcoming dependencies, adjusting to life or making changes.
  • Make recommendations for staff hires and discharges.
  • Train new support staff and new clinical employees and interns.
  • Provide prompt performance evaluations, clinical competency reviews and help staff define learning and training needs as well as the development of future goals.
  • Conduct regular, scheduled, clinical supervision meetings with each counselor and document.
  • Conduct regular team meetings and document.
  • Provide direct clinical service to a partial caseload, achieving 10 billable hours weekly.
  • Coordinate and approve staff schedules, time off requests and maintain Paylocity timekeeping records.
  • Ensure that both routine and emergency building/facility issues are promptly communicated with FGC or Harvey Facilities Management, as applicable, and that action has been taken to resolve the issue.
  • Ensure FGC Team participation in all fire and safety hospital-wide codes/drills.
  • As part of risk management, ensure clear concise written reporting of any incidents and forward to FGC Risk Management Officer.
  • Know and disseminate FGC policies and procedures, periodically reviewing key SOP areas with staff.
  • Contribute to new ideas and problem solving via managers' meeting.
  • Actively contribute to a culture of high level communication by clearly modeling active communication to staff and soliciting information and feedback from staff and sharing with the management team.
  • Supervising the Office Manager, ensure that documentation for FGC accounting, such as daily cash reconciliation, other accounts payable information, accounts receivable information, and service production verification (e.g. reporting of toxicology production, IL.DHS.DASA openings, etc.) is accurate and timely.
  • Provide crisis intervention as needed using appropriate de-escalation and crisis intervention techniques.
  • Provide community relations interacting as appropriate with other providers and community members (e.g. DCFS, local Police Dept., local businesses, treatment facilities, health and mental health service providers, etc.) and maintains active linkage agreements.
  • Serves as an effective role model for written and verbal communication.

Schedule Hours: Monday thru Friday 6:15am - 2:45pm; Saturday 7:30am - 10:00am
Work Location: 484 Lee St., Des Plaines, IL
Start Date: ASAP
Pay Rate: $45,000 - $55,000

Experience and Skills
Education and/or Experience: Master's degree in a behavioral healthcare field with licensure is preferred. Candidates with a bachelor's degree and 2-3 years of supervisory experience will be considered. Candidates must have one of the following; CADC, LCPC, LPC, PhD, MD, LSW, or LCSW certification/ licensure. Leadership and supervision skills are necessary to ensure staff and program development.
Certificates, Licenses and Registrations: State of Illinois Certified Drug and Alcohol Counselor (CADC); State of Illinois Licensed Professional Counselor, Licensed Social Worker required.
