Fraser is currently seeking a highly motivated and experienced individual to join our team as a Program Coordinator/Designated House Coordinator. Applicants with strong communication skills and experience coordinating services are strongly desired.

Qualifications:
Candidates must qualify based on one of the following three as assigned by 245D, which includes a combination of work experience and education.
- HS Diploma or Equivalent
- BA/BS in Human Services or related field with 1 year of full-time work experience(or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older.
- Associate degree in Human Services or related field with 2 years of full-time work experience(or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older
- A minimum of 50 hours of education and/or training related to human services and disabilities with four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older. Note: a qualifying Designated Coordinator under 245 D must provide Supervision.
- Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness and reflection.
Responsibilities:
- Assure necessary staff coverage, and recommend staffing, to meet the needs of the clients served
- Coordinate and/or complete all household repairs and other upkeep.
- Support other Program Coordinators with on call duties as directed.
- Coordinate the activities of daily living including personal care, hygiene, and grooming.
- Provide direct cares to our clients and supervisory duties.
- Collaborate with medical professionals to coordinate health and medical needs for each client served.
- This position is on-call and will serve as a part of an on-call rotation.

Location, Scheduled & Pay:
- We have various locations across the Twin Cities metro with various schedules.
- This position is typically Monday-Friday, standard business hours.
- House Coordinator - Starting pay $21 an hour and could go up depending on education nd experience with a $750 hiring bonus
- Program Coordinator - Starting pay $50,000 annually and could go up depending on education and experience with a $750 hiring bonus
