SUMMARY

The Program Coordinator for Nursing Services is primarily responsible for the management

and improvement of the Nursing Department and quality service provision to individuals

served, monitoring, and coordinating compliance with regard to all applicable federal,

state, and accrediting guidelines as well as company policy and procedure. This

individual is organized, efficient, and ensure follow through within a timely manner

related to all job duties. This individual must demonstrate consistent, thorough,

professional, and solution-orientated communication with a variety of internal and

external stakeholders.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Train staff in medication administration using the State of Indiana mandated curriculum.

2. Monitor medication administration.

3. Complete audits of monthly/quarterly nursing assessments for each resident.

4. Review/Maintain medical charts to meet applicable regulations. This includes 90-day physician orders, documentation of needed and required evaluations and treatment provided by outside service providers.

5. Ensure timely completion of annual physicals, bi-annual eye examinations, annual (or as ordered by dentist) dental examinations.

6. Assess residents' health care needs and participate in interdisciplinary decisions as to whether these needs can be met internally or require referral to outside medical/health care professionals.

7. Ensure Coordination and scheduling of medically related appointments with each Program Coordinator/Supervisor are completed timely.

8. Ensure attendance by assigned nurse for those appointments which require the nurse's input and others as group home scheduling requires. Also involves home visits to group homes for auditing purposes/incident follow up.

9. Assist in development of medical and health care procedures for the agency.

10. Provide health care information to the Program Coordinator for monthly, quarterly and annual reviews.

11. Provide and document annual Mantoux (TB) tests for consumers and staff.

12. Assist the Director of Operations in ensuring that the Federal ICF/MR standards and the State of Indiana group home regulations are met.

13. Assist the Director of Operations to ensure that medically related standards for Supported Living/Structured Family Caregiving are met.

14. Ensure the protection of consumer rights in the provision of health services.

15. Provide training to clients in health-related areas.

16. Ensure all verbal and written communications are presented in a professional manner, reflecting dignity and respect for clients and other co-workers.

17. Communicate with clients and their families, guardians, case managers, behaviorist, day programs, and any other individuals or entities associated with consumers in a manner that promotes positive interaction and understanding.

18. Actively participate in department and/or service site discussions and meetings as needed and take action on all assigned items.

19. Immediately alert supervisor/manager of any potentially critical issue or crisis situation.

20. Utilize discretion and sound judgment to provide the best care possible for clients.

21. Cooperative attitude and tolerance, particularly in a stressful situation or during a confrontation.

22. Maintain confidentiality, integrity, and trustworthiness.

23. Must Maintain Current Required Trainings and Certifications in CPR/Basic First Aid and Medication Administration.

24. Assist the Director of Operations with various administrative tasks to support department operations as directed; may also be assigned special projects.

25. Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon a client’s circumstances and/or agency and regulatory policy changes.

26. Meet on-going position requirements including training.

27. Participate in interdisciplinary team meetings, as needed.

28. Present in-service training to staff as requested by the Administration.

QUALIFICATIONS

1. A current Indiana Nursing License is required.

2. Previous supervisory experience required.

3. Knowledge of regulatory requirements and accreditation standards required.

4. Skilled in evaluation and assessment of individual's health needs.

5. Ability to learn and effectively utilize computer software applications to accomplish tasks.

6. Ability to read, comprehend, interpret and apply information from general procedures, instructions, data and reports

7. Ability to learn new concepts, which will be provided through departmental training sessions on an ongoing basis.

8. Must possess excellent written and oral communication skills with the ability to mediate conflict and problem solve.

9. Must be free from communicable disease with a negative TB test or chest x-ray

10. Maintain regular attendance and punctuality and accurately maintain attendance records.

11. Must have a valid driver’s license, registration and proof of insurance and acceptable driving record.

12. Must have the ability to complete all required trainings and certifications and demonstrate competency of skills taught.
