TITLE: Professional Student Clinical Rotation

Key Responsibilities:
Assessment of Health Status
- Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients, under the guidance of a SRCH Preceptor.
- Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, immunocompetence; presents case to SRCH Preceptor.
Diagnosis
- Works under the guidance of a SRCH Preceptor to make diagnoses of acute and complex chronic conditions.
- Under the supervision of a SRCH Preceptor, manages diagnostic tests through ordering, interpretation, and appropriate follow up.
- Formulates differential diagnoses by priority and presents these to SRCH Preceptor.
Formulates Plan of Care
- Under the guidance of a SRCH Preceptor, formulates and documents a plan of care to address acute and chronic health care needs.
- Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum.
- Under the supervision of a SRCH Preceptor, prescribes diagnostic strategies and therapeutic interventions (both pharmacologic, non-pharmacologic) needed to achieve expected outcomes.
Communication and Collaboration
- Maintains ongoing communication and collaboration with interdisciplinary health care team, especially with SRCH Preceptor.
- Facilitates and communicates with patient and family members to promote continuity of care across the outpatient continuum of care.
- Consult with SRCH Preceptor on the assessment and treatment of every patient case and per clinic policy and protocol.
Documentation
- Documents key components of patient's health history via progress note and electronic health record, under the supervision of SRCH preceptor.
- Documentation is timely and meets SRCH standards.
Professional Practice
- Demonstrates Professional Practice behaviors and follows SRCH standards for professionalism.
- Delivers well and chronic disease care with cultural humility to a diverse patient population of all ages.
