Kaiser Permanente/ The Southeast Permanente Medical Group is seeking a PRNPalliative Care Nurse (RN) who will be responsible for program activities, patient coordination and participation as an interdisciplinary member of the inpatient Palliative Care (IPC) team. The activities will include case finding, triaging, assessing, planning, teaching and coordinating care as necessary to respond to the patient's needs for Palliative Care. This person will also work with other members of the inpatient Palliative Care team to develop the most appropriate plan of care for members and will work with the team to coordinate care to ensure that the members needs and wishes are met. In addition, responsibilities will include 1) interacting with practitioners, members/families, IPC program staff, vendors and other Kaiser Permanente and quality resource management (QRM) staff, 2) education of practitioners, member/family and others, 3) providing emotional/psychosocial support to patient/family, 4) coaching family/patient on how to talk about difficult issues, 5) educating patient/family on disease and caregiving issues, 6) coordinating symptom management across all hospital shifts, 7) providing expertise in therapeutic medication for pain and symptom management and making recommendations, 8) monitoring quality of care, and 9) promotion of high level of member satisfaction.
This position will mainly be at Emory St. Joseph's Hospital and our Gwinnett Medical Office in Duluth.
The ideal candidate should have unrestricted current RN licensure in the state of Georgia or documentation to show intent to seek license, BSN, BCLS certification and three to five years of experience with chronically ill adults and clinical nursing experience in an area such as medical, home health or hospice nursing care.

Minimum Education and Experience Requirements:
- Experience in program development and policy and procedure development
- Well organized, detail oriented
- Willing to work with adults who are at the end of life with complex medical and social needs
- Functional knowledge of computers
Preferred Education and Experience:
- Knowledge of ICD9 and/or CPT4 coding beneficial
- Experience in hospice, Palliative Care or home health environment
