A Private Duty LPN is responsible for providing nursing care to patients in a private facility. The LPN will work closely with the patient's and other healthcare professionals to develop and implement a care plan that meets the patient's needs. The LPN will be responsible for administering medications, monitoring vital signs, and providing wound care as needed. The LPN will also be responsible for documenting all care provided and communicating with the patient's healthcare team as needed.
Responsibilities:- Provide nursing care to patients in a private facility
- Develop and implement a care plan that meets the patient's needs
- Administer medications as prescribed by the patient's healthcare provider
- Monitor vital signs and report any changes to the patient's healthcare provider
- Provide wound care as needed
- Document all care provided in the patient's medical record
- Communicate with the patient's healthcare team as needed
- Current LPN license in the state of Connecticut
- Minimum of 1 year of experience as a Private Duty LPN
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong critical thinking and problem-solving skills
- Ability to work flexible hours, including evenings and weekends
