General Statement of Duties


The Registered NursePrimary Care provides specialized transitions and/or palliative nursing care to identified Primary Care patients who will benefit from increased support and care to improve quality of life, manage and relieve symptoms, and improve psycho-social needs. Responsibilities include initial and ongoing assessments of patients, development and revisions of the plans of care, timely documentation and communication of assessments, and coordination of care with the palliative and transition care team.


Essential Position Functions


+ Makes initial and on-going visits to assess patient and provide baseline and comprehensive data. Utilizes nursing process to provide patient care that is individualized, goal directed and consistent with current standards of practice.


+ Identifies and communicates the needs of the patient and family to the interdisciplinary group which includes the PCP, assists with identifying goals of care, and monitors and documents progression toward goals.


+ Participates as a member of the interdisciplinary group to establish and implement a biopsychosocial care plan in patients with serious, complex and/or chronic illness.


+ Educates patients and families regarding disease process, pain and symptom management, and what to expect in the event of disease progression.


+ Counsels patients and families regarding end-of-life issues and provides guidance throughout the trajectory of illness.


+ Maintains high standards of professional practice by adhering to the ethics of the profession which includes seeking continuing education to support a goal of excellence in nursing care.


+ Relates to patients and families in an open, empathetic and supportive manner while maintaining personal and professional boundaries.


+ Provides hospitalized patient coordination, conducts urgent nursing visits, and obtains lab specimens as requested by PCPs.


+ Works closely with PCPs and Hospice team to ensure a continuum of care.


+ May include evening, night or weekend scheduling.


+ Completes all documentation and work within the scheduled work day.


+ Maintains standards of practice for patients enrolled in the Independence at Home Demonstration Project.


+ Responsible for documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards per policies and procedures.


+ Accepts and performs other duties as assigned

+ Maintain patient/family and staff confidentiality in compliance with HIPAA laws.


+ Participate in the coordination of the patient’s care in accordance with professional standards of practice.


+ Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.


+ Develop and maintain professional boundaries with all patients/family/caregivers.


+ Maintain agreed upon work schedule and meet deadlines for completion of work.


+ Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing facilities, assisted living environment, etc.


+ Serve as an ambassador for CareOregon and Housecall Providers at all times.


+ Demonstrates cooperation and teamwork and maintains collaborative relationships with all levels of staff and community partners.


+ Participate in orientation of new employees, volunteers or students seeking experience in palliative and transition care.


+ Attend staff and mandatory in-services meetings, as required.


+ Participate in ongoing education and assume responsibility for updating professional skills


+ Familiar and comfortable with a compassionate, person-centered approach to communication, education and care planning.


+ Maintain patient and personal safety regarding occupational health and safety and participates in safety program, as requested.


+ Participate in Quality Assessment Performance Improvement efforts to continually assess and improve quality of care, as requested.


Knowledge, Skills and Abilities Required


+ Demonstrates ability in case management, on-call triage management and nursing preferably in a palliative care or hospice setting


+ Knowledge of Federal, State and Local regulations, HIPAA privacy rules and regulations


+ Computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet


+ Excellent interpersonal skills


+ Ability to work both independently and collaboratively


+ Ability to work under pressure to meet deadlines


+ Ability to take initiative and utilize innovative techniques and ingenuity


+ Ability to interact with various departments throughout the organization


+ Ability to work flexible hours, if needed


+ Ability to manage conflict situations


+ Ability to plan, prioritize and coordinate multiple projects or tasks

Education and/or Experience


Required:


+ Current unencumbered license as a Registered Nurse in the State of Oregon


+ Minimum two years’ hospice, palliative/oncology, geriatrics and/or home health nursing experience


+ Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle


+ CPR certification at hire or within 6 months in position
