Job Summary:

The Post Permanency Coordinator will be responsible for ensuring compliance with Adoption State and County regulations for children, youth and families for whom APM has received a Statewide Adoption and Permanency Network (SWAN) referral. This person will provide case management pertaining to permanency services to the children/youth, permanency resource parent(s) and foster parent(s) in accordance with service description and SWAN contract and program guidelines.


Some of the main duties are:

  • Assessing the physical condition of the resource home
  • Completing Post Permanency Services in accordance with SWAN guidelines and benchmarks.
  • Complete all required Post Permanency assessment tools and documentation
  • Monitoring contractor progress on assigned services to ensure timeliness and escalate any concerns to supervisor
  • Maintaining up-to-date files, according to state and county requirements, and provide status updates on families/services
  • Assisting with training and social work services to program families
  • Must prepare for and attend regional meetings, statewide meetings and conferences, as well as service specific meetings and practice forums, court, etc. as required

Requirements:
  • EDUCATION: Completed Bachelor's Degree in Social Work, Psychology or related field
  • EXPERIENCE: Minimum one year working in the adoption system or other program with direct social service delivery (preferably in the Hispanic community).
  • Knowledge of PA Chapter 3700 preferred
  • Familiarity with Microsoft Office Suite
