Position Summary:

 

The Post Discharge Care (PDC) Coordinator is responsible for supporting the process that manages all urgent care/PHD encounters and high risk OB admissions – concurrent review and discharge planning.  In addition, the PDC coordinator is responsible for the daily checking and maintenance of the follow up grids for all campuses.  The goal of the PDC Coordinator is to effectively support the in-patient management process by assuring the right information is provided timely to the RMG administrative management team, hospitalists’ and care managers.

 

Essential Duties and Responsibilities include the following:

 

Customer Service

  • Anticipate our customer needs:  Understand the wants and needs of customers, listen for cues and identify how to respond and what level of intervention they need.           
  • Refrains from discussing personal business in front of the customer or speak a foreign language different from the customer’s language.
  • Greets patients, family members, and physicians in a positive manner by introducing self, speaking clearly and with confidence.
  • Returns phone calls to customers and follows up with requests.
  • Assists other staff and supports the team approach.
  • Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner.
  • Demonstrates honesty and integrity in everyday activities.
  • Protects privacy for both patients and employees; ensures all personal health information is kept confidential i.e., (sensitive papers, charts, and reports are not in view of the public).
  • Recognizes when an error has been made and immediately reports to appropriate manager.
  • Participates in “service recovery” through follow-up with an upset customer, gathering information, and demonstrates empathy.
  • Functions as liaison between care managers, patients, physicians, and other healthcare providers.
  • Communicates appropriately and clearly to management, co-workers, and physicians.
  • Communicates using age appropriate language.
  • Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs.
  • Maintains quality monitors as assigned by Supervisor.
  • Performs other duties as assigned
  • Participates in orientation, instruction/training of new personnel.
  • Pursues additional education (in-services, seminars, self study, and formal education programs) to advance skills and knowledge.
  • Participates in the efficient, effective, and responsible use of resources such as supplies and equipment.

CRITICAL ELEMENTS

  • High Risk OB Census will be verified every morning with regards to in house status and a review will be requested.
  • Authorizations for Home Health/DME/Follow up appointments will be done coordinating with Care Managers.
  • Check eligibility and benefits for all new admissions.
  • Mail letters, when applicable
  • Request for authorizations will be given via fax/return phone calls.
  • Maintain Follow up grids by checking telephonically and/or through Nextgen if the member showed to follow up appointments.
  • Work with PHD clinic schedulers to assist with DCP post D/C clinic appointment.
  • Filing closed cases should be done in a timely manner.
  • Communicate to RMG Medical Director or designee, immediately any admissions to non-contracted hospitals.
  • Provide face sheet to RMG Medical Director or designee, immediately upon receipt and eligibility check.
  • All new admissions are to be checked for Out of Area status.
  • Assist Inpatient Care Managers with planning for discharge by generating authorizations and scheduling appointments for services needed within 7 days of discharge.
  • Communicate with High Risk department any services identified upon discharge that is necessary beyond 7 days after discharge.
  • Support the safe, continuous delivery of medically necessary care and supportive ancillary services in the inpatient and transition to home care setting.
  • Identify network needs and report to management for potential contracting opportunities.
  • Review case statuses to ensure proper documentation and case closure. Work with assigned Care Managers to complete all documentation.
  • Assist staff with completion of retro claims case processing as necessary to assure timely turnaround times.
  • Prepare reports as assigned by Supervisor or Manager.
  • Assist the Medical Director with requests from primary care offices and specialist offices
  • Other duties as assigned by Supervisor or Manager

The pay range for this position at commencement of employment is expected to be between  $18 - $20 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

 

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements

Education and / or Experience:

  • High school graduate required; Associate in Arts degree preferred.
  • Knowledge of Healthcare and Managed Care preferred.
  • Typing 60 words per minutes with accuracy.
  • Knowledge of computers, faxes, printers and all other equipment.
  • Proficient in MS Office programs (i.e., Word, Excel, Outlook, Access and Power Point).
  • Ability to deal with responsibility with confidential matters.
  • Ability to work in a multi-task, high stress environment.
  • Medical terminology Knowledge
  • Good phone etiquette required.
  • Knows and follows the Employee Handbook policies and procedures.
