Job Summary:
The role of the Post Acute Provider is to provide high quality, cost effective, and evidence based medical care to patients in skilled nursing facilities. The Post Acute Provider is expected to provide clinical care for patients admitted at our affiliated skilled nursing / outpatient primary care facilities with Internal Medicine, Family Practice, General Practice or Geriatric Medicine. Join a well-established and growing group of physicians that treat patients throughout Central Florida. Average daily patient census of 20-25, Day call from 8 am to 6 am Monday-Friday and Rotating night call Schedule.
Primary Functions:
- Responsible for the assessment, evaluation, and clinical management of patients in the post-acute facility. This is inclusive of performing histories and physicals, as well as, conducting physical examinations.
- Diagnosis and active management of patient conditions, including evaluation and treatment, inclusive of daily patient care.
- Attend multi-disciplinary rounds at post-acute sites.
- Orders, conducts, and interprets diagnostic studies and laboratory tests
- Performs diagnostic and therapeutic procedures including but not limited to suturing, wound care, incision and drainage and splinting
- Prescribes or recommends pharmacologic and non-pharmacologic therapies, including controlled medications
- Consults appropriate physician(s) for patient care needs
- Refers patients to appropriate sources of care as required
- Assures the efficient utilization of resources while supporting clinical pathway implementation and compliance
- Assures appropriate and timely referrals to a tertiary care facility as indicated by the patient care needs in consultation with the primary care physician or attending physician
Education, Training, Experience:
- Doctorate degree in medicine (MD or DO).
- Completion of advanced graduate medical education in Internal Medicine or Family Medicine.
- Must be Board Certified or Board Eligible (IM or FP)
- Active FL medical license in good standing
- DEA license eligible/active
Knowledge, Skills and Abilities:
- Effective written, verbal, and electronic communication skills

- Ability to collaborate with colleagues to provide quality healthcare
- Ability to work under pressure
- Computer literacy
- Must be personable
Additional Qualifications:
- Prior experience using an electronic medical records system (EMR)
- Bilingual(Spanish & English) required
Relationship Reporting:
- Reports to the Medical Director
Physical Requirements:
- Communicate effectively with patients and staff
- Be able to sit, stand or view a computer screen for extended periods of time
- Perform repetitive hand and wrist motions for extended periods of time
- Be able to use hand and fingers to handle or feel
- Be able to reach with hands and arms
