Rate of Pay: $20.00-$25.00 per hour

Work Location: Cornwall, NY

General Purpose: The Population Management Coordinator will design and implement a comprehensive Population Management plan with the goal to improve patient health outcomes and increase patient encounter.


Description of Duties: 

  • Coordinates the Population Management initiative, including but not limited to: 
  • Running or requesting reports for preventative and chronic care services that are due or overdue for the patient population. 
  • Sending reminder letters according to Cornerstone Family Healthcare's Population Management plan. 
  • Conducts follow up phone calls to engage patients and make them appointments for overdue services. 
  • Monitors and maintains compliance for PCMH Population Health requirements 
  • Maintains a tracking system for success of the Population Management plan. 
  • Coordinates the Managed Care incentive programs including, but not limited to: 
  • Monitor patient compliance with the clinical measures set forth by each MCO 
  • Tracks reminders sent to MCO roster patients and initiates follow up phone calls if necessary. 
  • Maximizes incentives to the health center by appropriately managing tracking and follow up. 
  • Maintains relationships with representatives at each MCO and schedules regular follow up calls/visits with them. 
  • Validates data for reporting and Population Management purposes. 
  • Reviews data for consistency and accuracy. 
  • Makes recommendations for additional training for staff members as a result of data review. 
  • Makes recommendations for clinical EMR form modifications to improve data collection. 
  • Acts as a liaison with vendors engaging in patient reminder activities on behalf of the health center. 
  • Be familiar with HIPAA laws and Cornerstone Family Healthcare confidentiality policies and ensure that they are followed by Medical Records employees. 
  • Attend and participate in monthly department, All Staff and other required meetings. 
  • Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations. 
  • Perform other related duties as assigned. 

Requirements

  • High School Diploma/equivalent; Associates or Bachelor Degree in Public Health, Nursing, IT, Computer Science Highly Preferred
  • Highly organized with excellent oral and written communication skills 
  • At least 1 year experienced in working at a health center or hospital setting.
  • At least 1 year experienced with working with health insurance (e.g., MCO, MDCR, HMO)
  • Experience with different EMR systems (e.g., ECW, EPIC, Centricity, and other EMR systems)
  • Bilingual in Spanish highly preferred
