Description:
- Leveraging all available tools – including Electronic Health Record and insurance company portals, to provide direction for targeted outreach to patients
- Providing direction to a team of people as well as marketing on the messaging and focus for different campaigns
- Collaborating with leadership on the identification of quality care improvements
- Analyzing all available opportunities for high value impact on the business and community health
- Supporting supplemental data submission process for plans without electronic supplemental feeds
- Sharing findings of identified concerns or improvements related to gap closure, ordering process, and documentation with management and participating in the process improvement of the workflow
- Preparing reports as needed for internal and external customers
- Updating EHR and insurance company portals as appropriate
- Outreaching to outside health providers for results and quality data as needed to improve practice scores

- Bachelor’s degree or higher (Master’s Degree Preferred) in relevant Public Health, Healthcare Administration, or Human Services field.
- Must possess a combination of at least 1 year of public health work experience or relevant education, and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties.
- Ability to travel extensively within assigned communities.
