Overview:
The Population Health Coordinator partners with patients, providers, and other members of the clinical care team to implement standard work processes that drive improvement in quality and patient outcomes. This role will apply value-based care standards and utilize standard analytics tools to drive performance.
This position will work fully remotely, but we prefer the ideal candidate to live in Arkansas, Missouri, or Oklahoma.

Qualifications:
- Experience: 3+ years clinical healthcare experience.
- Required Education: High school diploma or equivalent.
- Preferred Education: Graduate of a Medical Assistant program
- Certifications: Medical Assistant Certification preferred.
- Other: Preferred background in medical assisting, social work, or education. Experience with EPIC scheduling and clinical workflows preferred.
