Dancing Sky Area Agency on Aging are looking for a self-motivated, innovative person that enjoys working on new projects. This position is time limited ARPA grant funded.
We would be looking at supporting care planning work for older adults (65+) that need care support to reduce their risk of maltreatment and support safe independent living. Strong skills in planning, organization and follow through in a high paced changing environment. Strong computer skills. Travel and overnight stays may be required within 21 county planning and service area.
Specific responsibilities:
* Tracking of project and persons served.
* Outreach and relationship building to launch project.
* Support planning skills for individuals and families.
* Strong understanding of aging services.
* County coordination and support in building referrals.
* Other duties as needed to complete the pilot project.
Experience Required:
Preferred bachelor's degree from an accredited college or university in nursing, social work, gerontology, or related human services field. Experience in care coordination and case management. OR a minimum of three years’ work experience is preferred.
